LETTER OF UNDERTAKING
I DIEKOLA TAIWO ROSANAH with matric Number 201117 hereby undertake that I am
guilty of not meeting up to 75% of nursing class attendance. I hereby tender my apology.
The reason for my shortcoming was due to the fact that I was indisposed for 2 weeks of which
lectures were held. I informed the class Rep but he forgot to inform the lecturer in-charge (Mrs.
Olajide).
My action was not intentional and in view of this, I hereby tender my apology and I promise
that this will not repeat itself.
I promise, henceforth, to abide with the rules and regulation of the department, faculty and
institution at large.
