III. Procedure

A. Introductory Activities
1. Check the Assignment
2. Review: Let student answer what it is focused group discussion.
3. Motivation: There will be a game for the class entitled “survey says”.
Five questions will be flashed to the screen and let students
manipulate the laptop to choose the answer. The questions and
answers were taken similarly from “Family Feud” (ABS-CBN
gameshow)

B. Development of the Lesson


1. Activity
Group the class into 2. Each group will be tasked to brainstorm their ideas on
how to formulate a questionnaire for a survey.
2. Analysis
1. How did you formulate your questionnaire design?
2. What are the rules in making Questionnaire Design?
3. Abstraction
There are some basic rules that have to be followed in question formulation.
These are:
● The questions, I their totality, should be able to elicit all the
necessary information required in the research
● Each question should be clear and definite
● Each question should cover one topic at a time.
● Each question should be presented in a neutral manner.
● Each question should be translated into the dialect that the
target respondents are familiar with
Thorough discussion on each rule will be presented to whole class.

Generalization: What are the basic rules in formulating questionnaire design for
survey?

4. Application

There will be a game for the class. All they have to do is to raise their hand if
the statement is true and left hand if the statement is false. It would be
three statements.
1. In the survey questionnaire each question should be presented in neutral
manner.
2. In the survey questionnaire each question should cover a lot of topic at a
time.
3. In the survey questionnaire each question should be clear and definite.

IV. Evaluation
Give at least 3 rules in formulating questionnaire design for a survey and explain
briefly.

V. Assignment/Agreement
Create your own survey questionnaire following the rules in formulating
questionnaire design for a survey. Take note that it should be related on the
product or service you are planning in your business plan.
