Action: In this week, the task is to discussed the health care services in each member of
the family and educate the parents about the sanitation in their surroundings. Also, we
encourage the family to become more cautious with their health.

Reflection: As a community health nurse, we should become more flexible to help and
educate the people in our community. We should educate them especially in their health
on how to care and become healthy.

Faith: 1 Thessalonians 5:11 "So continue encouraging each other and building each
other up, just like you are doing already." This biblical passage is very meaningful to all
people and especially to me as student and future nurse. We must continue and keep
encouraging each other and building up each other because this may help us to have
more confidence and if we keep on encouraging and building up each other, we may all
have brighter future. Forget about the negatives and let's just keep on encouraging and
building up each other because it is also for the sake of everyone.

Action: As a future nurse I realized that when helping other people you must help
others with open heart and also if ever, I encounter this kind of situation again I will
surely try my best to work well and give the best care and love to my patient. this well
serve as a lesson and experience to be one of the great nurses someday

Action: Our clinical instructor gave us time to do our session module that we will be
needed in the upcoming virtual meet that we are conducting. We formulate questions
that are related with our topic.

Reflection: In our topic mental health, we create questions that are necessary to give
us knowledge about how they manage their mental health and how each person seek
help with their condition. Mental health is should be given focus because it is as
important as our body.

Faith: “Carry each other’s burden, and in this way, you will fulfill the law of Christ”-
Galatians 6:2

Action: One of our responsibilities as student nurses is to provide care and comfort to
others. When we know someone who is suffering from mental illness, we should be
the ones to offer them any and all means of reassuring them that they have a friend
who will always be there for them. We should give them the impression that they
have a listener, regardless of what they want to say, and we should not invalidate
their feelings. With this, we not only performed our task, but we also fulfilled the law
of Christ by doing with all of our hearts.

Action: Last day of our duty but we are still assigned to monitor the health of the
family. Also, we shared all the knowledge we gather since day one and be able to
encourage the family to become more cautious with their health.

Reflection: - From the very start of my journey in nursing, I was worried because
there was something stuck in my mind that I couldn't express verbally. These questions
like “what if I fail, what if I cannot, what if it is not for me.” These questions have been
stocked in my mind until now. But what made me share this, or what’s the point of this,
is that, as I interviewed different clients and families, I felt like I am happy with what I
am doing. I felt like I had always taken initiative when I interviewed different clients. I
believed that regardless of the pressure we experienced like sleepless nights, unable to
attend family bonding because of full loaded activities are all worth it and fulfilling as
long as we are happy with what we are doing and we see our clients getting better and
have achieved self-satisfaction, and that’s what really matters. And most importantly,
because of the clients, I found where my heart wanted to be.

Faith: Colossians 3:23–24 Whatever you do, work heartily, as for the Lord and not for
men, knowing that from the Lord you will receive the inheritance as your reward. You
are serving the Lord Christ.

Action: In order for us to be an effective Nurse in the future, we should always prioritize the
health and safety of our patients. We are not in the right position to scold them or neglect the
patient for the right in receiving the proper treatment intended for them. As future a Nurse, I
should instill the value of patience and courage to face patients who might not cooperate well

Action: Our Session Module will be presented this week, and our Clinical Instructor will point out any modifications
that need to be made. I was able to effectively report my contribution. Because this is our final week of community
service, I was able to visit with my clients one last time. My community files, such as my DBHS, BP logbook, and
photo documentations, were completed by myself. I offered them guidelines and encouraged them to maintain their
good eating habits and lifestyle.

Reflection: Even though this is our final week on duty, we will continue to keep an eye on them and provide service
to our community. Because being a nurse has such a broad scope, we must constantly remember to be prepared at all
times and in all situations.

Faith: “The Lord himself goes before you and will be with you; he will never leave you nor forsake you. Do not be
afraid; do not be discouraged”-Deuteronomy 31:8. As nurses, we should look up to Jesus and follow in His footsteps,
because He came to serve, not to be served. As nurses, we have a responsibility to serve a large number of people. We
must provide unselfish assistance and unwavering support to individuals who are in desperate need of medical care.
Our service should not be limited.

Action: If I ever become a community nurse, I will be able to put all of the skills and information I've learned
through our many activities and projects to use. As I begin a new chapter as a nursing student, this will serve as my
background. In the future, I will be more diligent and responsible in everything I do, particularly in keeping accurate
community files, paper work, and nursing interventions.

Action:  To be assessed whether we completed the FNCP correctly and produced the
appropriate nursing care plan for our client, we must present our completed FNCP to
our clinical teacher this week. Some of us didn't identify key variables, while others
used medical jargon that we weren't allowed to use at the time.

Reflection:  A successful intervention may not be possible or we may have more


difficulty performing it since you missed one of the FNCP components during the
presentation. It was also difficult because we had to keep track of our goals and educate
the family about them.

Faith:  Proverbs 3:5-6 " Trust in the with all your heart and lean not on your own
understanding; in all your ways submit to him, and he will make your paths straight.

Action:  As future nurses, we must have a great deal of patience and not allow our
anger to control our actions. Patience, I believe, is the key or secret to being a better
nurse in the future. Patience entails demonstrating to our patients that we, as
prospective nurses, have received excellent training from our lecturers and mentors.

It's our last day on duty, and we've been assigned to continue monitoring the supply of basic
health services by our two clients, as is customary. Two of these things are checking their vital signs and
monitoring their blood pressure. I was able to provide them with both health education and previous
knowledge by doing so. They need to look for themselves and maintain a healthy lifestyle.

Reflection:
Community nursing is concerned with promotion of health, illness prevention, and health management.
The backbone of community-based nursing is evidence-based care. Information about the population's
issues, situations, and strengths is gathered, analyzed, and acknowledged through community
assessment. My past nursing experience has improved my growth as a student nurse and given me more
confidence in the care I deliver. Ideally, others will gain as much value from this as I have. My personal
and professional development will empower me to mentor and promote the learning and development of
other nursing students in the future.

Faith:

2 Corinthians 4:17 For momentary, light affliction is producing for us an eternal weight of glory far
beyond all comparison.

Action:

As God's children, we have been promised that His grace is sufficient to deal with every eventuality we
may meet in life. We need never endure our affliction alone, for He has promised to be with us, and God
is faithful, Who will not allow any of us to be tried beyond what we can bear. There is no affliction that
we have to face alone, for He has promised to provide us with the sufficient strength we need to endure
it.

Action: It is our last duty day and as usual w e were assigned to continue monitoring our
two customers' delivery of basic health services. Checking their vital signs and
monitoring their blood pressure are two of these things. By doing so, I was able to
present them with both health education and past knowledge. They must take care of
themselves and live a healthy lifestyle.

Reflection: Nursing in the community focuses on health promotion, disease


prevention, and health maintenance. Evidence-based care is the cornerstone of
community-based nursing. Through community assessment, information on the
population's problems, conditions, and strengths is obtained, analyzed, and
recognized. My previous experience has aided my nursing development and led to my
greater confidence in the care I provide. Hopefully, others will benefit from this as
much as I have. In the future, my personal and professional growth will enable me to
guide and encourage the learning and development of other nursing students.

Faith: Philipians 4:13: “I can do all this through him who gives me strength.”

Action: Nursing is difficult, which is why I chose this verse as my power verse. I'll
never have all the answers, spot every problem before it turns into a major problem,
or get everything perfect. It's simply too complicated a situation to expect perfection
from oneself. But I know I can count on the perfect one. As I go about my day, I can
pray to hear His voice and commit to trust the Lord to provide me with the resolve I
need to complete my job successfully. He can help me when I can't. All things are
possible for me because of Christ, who gives me strength.

Reflection: We will continue to keep an eye on them and provide assistance to our
community even if this is our last week on duty. Because a nurse's job entails such a
wide range of responsibilities, we must always remember to be prepared at all times
and in all situations.

Faith:  Each of you should use whatever gift you have received to serve others as
faithful stewards of God’s grace in its various forms. 1 Peter 4:10
 Maybe God's gift to me is to become a successful nurse so that I can help the
community with my ability and talent.

Action: This activity taught me how helping is important and made me realized that

Action: My motivation for getting up every day to help these individuals is to become
a prominent person in their lives and inspire them to protect their health for the sake
of their loved ones. Even if it is stressful to carry out the activity due to the
circumstances that we may encounter along the road. Whatever occurs, we put on a
happy face to show them that we genuinely care about our task. We provide our
clients with basic health services in order to check their health state. And I provided
them nursing interventions to either avert the danger or to improve their current
status.

Reflection: As community health nurses, we prefer to express our sympathies to


these people in the community by performing the work that they deserve. These
tasks have taught me more about how to care for others around us while not
expecting anything in return.

Faith: Philippians 2:4, "Instead of each person watching out for their own good, watch
out for what is better for others. “This bible quote says that it's appropriate to
prioritize ourselves first, but we should also do things that benefit our community. We
should all work together to accomplish the success that we are all striving for, and we
should all have faith in one another for the greatest possible outcome.

Action: The lessons I learned while doing these duties taught me how to manage
similar situations in the future if they arise. I'm optimistic that it will be a memorable
experience in my nursing career. And I learned a lot about myself, notably that I can
accomplish more. That I will be able to confront it as a mature and grow into a

Action: This week, we finalized our session module, and thankfully, it went well.
Another activity was given to us, and we needed to construct questions for the Training
Needs Assessment. The members were divided into three groups since we had to
formulate questions based on the possible clients' knowledge, skill, and attitudes.
Before ending the day, 20 questions were made and sent to our clinical instructor to be
checked.
 
Reflection: Whenever we're doing something that needs participants, we should
consider what they know and what they can do to have an effective and efficient output
not just because we want to do something, we have to do it right away without asking
the people involved.

Faith: "Don’t act out of selfish ambition or be conceited. Instead, humbly think of


others as being better than yourselves.  Don’t be concerned only about your own
interests, but also be concerned about the interests of others.” Philippians 2:3-4

Just what I've said, we also need to consider others' perceptions, knowledge, or ability
about something before pushing something to carry out. This activity is not just for our
benefit to pass an output or get good grades, but it is to help those people who are not
doing well at present. We should be someone who will support and encourage them to
be better and make them feel they are not alone.

Action: What I previously experienced or encountered will serve as a lesson for me to


take appropriate measures for future scenarios.
