weekly formative tracking for learner’s weekly achievement.
All teachers are ready with the with the tracking tools to
ensure that learner’s weekly formative performance is
monitored.
Since weekly tracking is available, there is a need to
check teachers’ provision of feedback to learners.

4. Tracked Teachers Preparation
and Distribution of Diagnostic
Tests with TOS
100% of the school heads tracked teachers’ preparation and
distribution of diagnostic test. The diagnostic test followed
the contextualized Division Policy on Assessment as to
distribution of items across grade levels. However, not all
teachers designed TOS in their diagnostic test.
School heads must ensure that teachers must
prepare the TOS so that alignment of test items and
competencies are ensured. Aside from TOS, teachers
must also prepare an analysis of the competencies
that were found low from the diagnostic tests.
