Training and Placement team would appreciate if you would take the time to complete this
form. Your input is very important to the future development of Placements in CJITS. The
purpose of this form is to assist in student placement experiences in an effort to ensure
high quality placement experiences for current and future out going students.

Training and Placement Coordinator will collect this data and review accordingly and
confidentially. All of the feedback obtained will be summarized and categorized. A generic
email providing a comprehensive overview of such feedback will then be sent to all relevant
placement sites. The results of this survey may also be used for further feedback to other
sources the development of Placements.
