This is certified that clinical study namely “A Pharmacognostical study on Shati

for its identification and to evaluate the clinical efficacy of Karchura (Curcuma

Dravyaguna under the supervision of Dr. Navneet Sharma is well defined as per texts
of Ayurveda and will not cause any inconvenience to the patient. The reference of the
drug under trial and there therapeutic indication under taken in the present clinical trial
are well explained in Ayurvedic literature.
The patient and the parents/guardian will be explained about the nature of
disease and clinical trial. They shall be explained the need of relevant investigation
required for diagnosis in the language they understand. They will also be explained
about the treatment modality they will receive and the duration of the treatment.
During this trial the patient will not be subjected to any additional procedure or
expenditure and the study will be conducted on only those patients who will give their
consent. The study is purely designed to create the evidence based efficacy of the trial
drug.

Brief resume of intended work

7. Introduction
 Ayurveda is traditional holistic healing system of Indian medicine which
depends on plant support to form major chunk of its medicines.

 The aim of Ayurvedic science is to maintain the health of healthy individual and
to cure the diseases of the patients, which can be achieved by use of Ayurvedic
herbs which offer a variety of health benefits, including improving physical and
mental health and keeping the mind, body and spirit in balance.

 This ever vibrant science of life ‘Ayurveda’ has bestowed us with innumerable
number of herbs which have great potential in managing various types of
ailments, some of which were considered impossible to treat by other fields of
medical sciences and have shown marvelous results.

 Because of above reason, Ayurvedic herbs have become very popular in last few
decades and gaining more interest of modern world.

7.1 Justification for the selection of drugs for the Pharmacognostical study :

 Shati (Hedychium spicatum Ham-ex-smith) also known as Kapurkachari is a


perennial tall stout herb with fleshy rhizomes, thick straight stem with broadly
lanceolate leaves. The rhizomes have a strong aromatic odor and bitter taste and
its extract has been reported to contain essential oil, starch, resins, organic acids,
glycosides, albumin and saccharides, which has been advocated for blood
purification and treatments of bronchitis, indigestion, eye disease and
inflammations and possess anti-asthmatic, hypoglycemic, vasodilator,
spasmolytic, hypotensive, cytotoxic and antimicrobial properties.

 Whereas Karchura (Curcuma zedoaria. Rosc) also known as white turmeric, is


also a perennial rhizomatous herb composed of an upright pseudo stem, a corm
and underground cylindrical branches or rhizomes and fleshy roots. From March
to April the axillary buds of the corm and apical buds of the third-order rhizomes
emerge above the ground as inflorescences. This basal flower spike, which grows
about 30 cm tall, appears just before the foliage. On the node closest to the flower
spike, a vegetative shoot always develops. No additional floral buds sprout but
more vegetative shoots develop. New branches start to develop on corms of
recently formed aerial shoots. By autumn, the above-ground foliage dies back.
Various parts of this plant are used in Ayurveda and other folk medicines for the
treatment of different ailments such as diarrhea, cold, cough, cancer, flatulence,
dyspepsia etc. The species also possesses analgesic, anti-pyretic, anti-allergic,
hepato-protective properties and anti-inflammatory properties.

 Both the plants are Himalayan species belong to same family Zingiberaceae,
distributed wildly in Himachal Pradesh, Uttarakhand, Nepal and Sikkim and both
have Shwasahara Karma as per Ayurvedic classics but in comparison to each
other, Shati is well established Shwasahara drug.

 Because of same family, both plants share similar morphological characters that
create confusion in their identification and because of this confusion, both drugs
are used as substitute to each other in the market. Moreover there is no proper
authentication of both drugs. So with this purpose, a pharmacognostical study is
selected for their identification to standardize the authenticity of both drugs for
further use in future.

7.2 Justification for the selection of disease for the Clinical study :

 Life of Human beings mainly runs on three “B’s”. First BRAIN, second BEAT
and third last but not least BREATH i.e. Shwasan.

 Respiration is an important physiological process necessary for the existence of


life. In Ayurveda the functions of Prana Vayu is held responsible for the process
of respiration in human body. The channels or tracts (Srotas) in which Pranavayu
flows are called Pranavaha Srotas.

 Any disturbance in this process may lead to Shwasa Roga. Tamaka Shwasa is one
of the important disease of such disturbance of Pranvaha Srotasa. ‘Tamaka
Shwasa’ has two component words. The term Shwasa refers to both physiological
and pathological state of respiration and the word Tamaka indicates the specific
variety of the Shwasa Roga and emphasizes specific symptom ‘darkness in front
of eyes’ or ‘tiredness of eyes’ without much strenuous work to the organ.

 Thus the nomenclature ‘Tamaka Shwasa’ clearly indicates the disease where
difficult breathing associated with feeling of merging in darkness. It has the
predominance of Vata and Kapha Doshas and mentioned as Yapya Vyadhi i.e. a
disease of chronic nature in Charaka Samhita, while Sushruta considered it as
Krichchra Sadhya Vyadhi.
 Acharya Charaka quoted that “Shwasa & Hicca are two diseases, which are
much fatal to life amongst all other diseases” as mentioned below -

तस्मात् साधारणावेतौ मतौ परमदु र्जयौ। (च.चच.१७/९)

 This indicates the importance of need of Shwasa Chikitsa.

 As per various survey suggest that in Himachal Pradesh, diseases related to upper
respiratory tract like Kasa-Shwasa-Hicca has more prevalence as compare to
other diseases. This is either because Himachal Pradesh considered as Anupa
Desha (mountain climatic condition) where there is predominance of Kapha
Dosha, thereby people are more susceptible to diseases related to Kapha Dosha
or due to excessive use of tobacoo and smoking.

 Because of above mentioned reasons, Tamaka Shwasa is a very common disease


and it affects most of the population of this area.
 In current modern pathy, there is no satisfactory result for this. So keeping these
facts in mind, the present disease is being selected for study to provide a suitable
remedy for this problem and to revalidate the efficacy of Ayurvedic medicines
against modern pathy.

7.3 Justification for the selection of drug for the Clinical study :
 Our Acharyas have listed many herbs which have Shwasahara action. The
Shwasahara drugs include respiratory tonics and naturally occurring broncho-
dilators and immuno-modulators. Karchura (Curcuma zedoaria.Rosc) is one such
herbal drug, which acts as Shwasahara as mentioned in classics and it is
abundantly available in Himachal Pradesh and cost effective which may satisfy the
need of present day situation by acting as potent herbal bronchodilator but
clinically its efficacy on Tamaka Shwasa as Shwasahara drug, is still not popularly
known as compared to Shati. So with this perspective, the drug Karchura
(Curcuma zedoaria. Rosc) is selected to evaluate the clinical efficacy in Tamaka
Shwasa.
7.4 Aim and Objectives
 To review the classical and modern literature of Shati (Hedychium spicatum Buch.
Ham. ex. Smith) and Karchura (Curcuma zedoaria Rosc.).
 To carry out macroscopic and microscopic study on Shati (Hedychium spicatum
Buch. Ham. ex. Smith) and Karchura (Curcuma zedoaria Rosc.) for its
identification to standardize the trial drugs.
 To prepare standard quality medicine as per GMP & classical Ayurvedic texts.
 To review the literature of Tamaka Shwasa, as per classical references.
 To evaluate the effect of Karchura (Curcuma zedoaria Rosc.) in Tamaka Shwasa.

8. Review of literature
 A detailed review of Shati and Karchura will be done from Ayurvedic
literature, journals, internet and recent research works for its identification.
 Ayurvedic and modern literature will be reviewed thoroughly to know Tamaka
Shwasa in detail and its treatment, according to Ayurvedic therapeutics.

8.1 Previous work done on Karchura - Yes


 Vijay Kappathanwar, Roshi Mahajan , Paramjeet Puri , Rekha Sharma and
Shipra Sharma. “A research article on Evaluation of Shwasahara Effect of
Karchura (Curcuma zedoaria Rosc) with special reference to Bronchodilator
Action, published on 10/01/19, Int J Ayu Pharm Chem 2019 Vol. 10 Issue 1
page number - 346” - In this experimental study, the test drug Karchura in its
two different forms viz. Mula Churna and Mula Kashaya were taken, whereas
Churna form of the test drug Karchura yielded better results as compared to
Kashaya form.

 On the basis of above references, the trial drug has been selected for present
research work.

9. Materials and method :


9.1 Source of data:

1. Literary source: All classical literature and latest information like website will
be reviewed and documented.

2. Source of plant: Plant will be identified authentically and collected from the
natural sources.

3. Pharmacognostical laboratory

4. Clinical study will be carried out in patients suffering from the Tamaka
Shwasa.

9.2 Method of collection of data:

 Collection of the drug: Shati and Karchura will be freshly collected, found in
mid hill zone of Distt. Kangra (H.P.).

 Pharmacognostical study: The drug will be identified and collected


authentically from the natural sources and will be taken for the detailed
macroscopic and microscopic study of trial drugs (Shati and Karchura).

 Formulation: Karchura Churna of raw drug to be prepared.


 Preparation of drug: The selected trial drug will be collected locally and after
proper verification by the experts of Deptt. Of Dravyaguna, Churna (powder
form) of the dried rhizomes will be prepared under the guidance of Deptt. Of
Rasa Shastra Evum Bhaishjya Kalpana in the college pharmacy.

 Clinical study: After preparation of the trial drug, clinical study will be done
on the patients presenting with symptomology of Tamaka Shwasa selected
from OPD and IPD of R. G. G. P. G. Ayurvedic College and Hospital, Paprola,
Distt. Kangra (H.P.), irrespective of caste and religion.

10. Protocol of research


 ICE Approval: The ethical clearance will be taken for the research from
Institutional Ethics Committee (IEC) before starting the study.

 Consent of patient: Awareness about the duration and nature of the trial and
written informed consent will be taken.

 Registration of the patients who fulfill the inclusion criteria.

 Investigation mentioned will be advised to the patient before and after the
treatment.

 Patient information sheet: The detail about the nature of disease, various
aspects of clinical study will be shared with the patients and their quires will be
addressed.
 Case Record Proforma: The detailed case record proforma will be prepared
for the assessment of the patient before treatment and after treatment.
 Data so available and deducted clinically will be statistically analyzed.

11. Protocol for clinical study


 Sample size and grouping of patients - Study will be conducted on 30
patients suffering from Tamaka Shwasa and assigned as single group.

 Criteria for selection of patients:

 Inclusion criteria -Patients fulfilling the following criteria will be included


in the trial-

a) Patients willing for trial


b) Age - 16 - 70 years

c) Gender - Both Male and Female

d) Patients presenting with classical features of Tamaka Shwasa as mentioned


in Ayurvedic classics-

 Shwasa Kricchrata (Difficulty in breathing)

 Asino Labhate Soukhyam ( Relief on sitting posture)

 Kasa (Cough)

 Nistivanam (Phlegm)

 Ghurghurukam (Wheezing sound)


 Peenasa (Watery nasal discharge)

 Exclusion Criteria -

a) Patients not willing for trial.

b) Known case of any major chest pathology

c) Patient aged less than 16 yrs. and more than 70 yrs.

d) Asthma due to other systemic disorders, and other respiratory disorders.

e) Asthma in pregnancy.

f) Occupational Asthma.

g) Emergency condition of the patient, who requires oxygen inhalation.

 Dose of drug and Route of administration

o Orally, 1gm of Churna (powder) form twice daily.

o Duration of trial- 4 weeks

 Follow up

o There will be a follow up after 7, 14, 21 days during treatment and one after
completion of trial.

12.Criteria for assessment


 Effect of therapy will be assessed according to improvement in signs and
symptoms of Tamaka Shwasa.
 Depending upon subjective and objective parameter, assessment of response
will be made based on Gradation index as mentioned below, before and after
treatment -

13. Laboratory investigations

 Estimation of Hb%, TLC, DLC, ESR and FBS will be carried out for general
assessment of patients and exclusion of other pathologies. After treatment
investigation will be done in those cases where before treatment hematological
values are found abnormal.

 Absolute eosinophil count

 Chest X-Ray P.A. view

 Sputum for A.F.B.

 To rule out respiratory conditions or infections and confirm clinical


diagnosis.

14. Diagnosis criteria –


 The signs and symptoms of Tamaka Shwasa like Shwasa Kricchrata, Asino
Labhate Soukhyam, Kasa, Nistivanam, Ghurghurakam, Peenasa - mentioned in
Ayurvedic texts and objective laboratory investigations mentioned above, will
be the criteria for the diagnosis.
15. CRITERIA FOR OVER AL L EFFECT OF THERAPY:
 Patients will be assessed before and after the treatment for improvement in
symptoms on the basis of above said scoring pattern and percentage of
improvement will be calculated.

 Result : Final result based on clinical study, will be statistically evaluated and
categorized according to the following grades:

Complete 100% Relief in the Subjective and Objective


Remission Parameters.
Marked More than 75% Relief in the Subjective and Objective
Improvement Parameters.
Moderate 50% -74% Relief in the Subjective and Objective
Improvement Parameters.

Mild Improvement 25%-49% Relief in the Subjective and Objective


Parameters.
Unchanged Result below 25% considered as unchanged.

Purpose of the study – This is a research study being conducted by research scholar
Dr. Shefali Rana under the guidance of Dr. Navneet Sharma to prove the efficacy and

Procedure of the trial – If you are agree to take part in this study, you will be given
Karchura Churna two times orally in 1gm dose for period of 4 weeks.

You have to undergo investigation before and after the trial for assessment.

Expected side effects - The trial drug as such involves no side effects but yet
individual specific variations may occur at any time during the course of clinical trial.

If you want to ask anything about the clinical trial, you are free to ask at any stage of
trial. A copy of the information sheet and signed consent form will also be given to
