COMES NOW, the Petitioner Samson Adriano Delos Santos and unto this Honorable
Office, respectfully state:

1. That the Petitioner is of legal age, Filipino, widower and resident of Sta.Cruz, Manila;

2. That the Petitioner is the registered owner of a parcel of land covered by TCT No. T-
170579 registered in the Registry of Deeds of Guiguinto, Bulacan with an area of
THIRTY NINE THOUSAND TWO HUNDRED TWENTY NINE (39,229) SQUARE METERS,
more or less;

3. That the said title is covered by an encumbrance of the Provision of Section 4, Rule
74 of the Rules of Court;

4. That the Petitioner is requesting to cancel the encumbrance and declaring that the
foregoing had already lapsed and expired from the two (2) years period thus the
necessity of cancelling the said annotation;

5. That the reason of this Petition is to be able for the Petitioner to sell the property or
to do any related matter and for any legal purposes it may serve him.

PRAYER

WHEREFORE, it is respectfully prayed of this Honorable Office that the cancellation

of the said encumbrance be granted in favor of the Petitioner.
