JOINT AFFIDAVIT OF COHABITATION

WE, PEDRO R. MADRIGAL and ALMA K. SANTOS, both of legal age, Filipino, single and
both residing in 123 Rizal St., Brgy. Poblacion, Marilao, Bulacan, after having been duly sworn
to in accordance with the law, depose and state that:

1. We have been living together as husband and wife under the same roof, continuously and
without any interruption, since January 1, 2016 up to the present time, or for a period of
more than five (5) years;

2. During the period of our cohabitation and until the present time, neither of us has entered
into marriage with other persons, hence, there exists no legal impediment for us to marry
each other;

3. During our cohabitation, our union produced three offsprings, namely JULIETA S.
MADRIGAL, PEPA S. MADRIGAL, and BRUNO S. MADRIGAL, all of whom were
born on January 30, 2017;

4. We are executing this affidavit to attest to the foregoing facts and for the purposes of
contracting marriage without the need of securing the marriage license pursuant to
the provisions of Article 34 of the Family Code of the Philippines. 

IN WITNESS WHEREOF, we have hereunto set our hands this 1st day of January, 2022, in
Marilao, Bulacan.
