Description of Service Shortcoming


There was a fire in my office on 15 September 2011 and destroyed my Desk, laptop, printer and my
external hard drive.

Detailed Cause Analysis


Multiple adapters were used in the one plug which caused an overload in the adaptors and the
plastic started to melt and then caused a short circuit by which the wires caught fire and then the
wooden furniture.
The trip switch did not “trip” and thus the electricity supply was not cut off when the overload
occurred.

Improvement Plan/s
The building was designed that each office do have only one plug for electricity supply and thus
more plugs cannot be accommodated.
Make use of multiplug adaptors with an overloading protection built in the adaptors, the make is
SABS approved ABC 202.
ABC 202 adaptors will be ordered on 23 September 2011 and once delivered the adaptors will be
issued to all staff.
The main electricity boards’ switches will be tested bi-annually and if found defective the switch will
be replaced. The first inspection will be 7 October 2011.
New equipment was ordered on 19 September 2011.
