Amitryptiline Dose: initially 25mg Bd (daily dose of 10mg-25mg In elderly), if required to max
75mg Bd; initially 10mg nocte ing if required to 75mg Nocte.
Atomoxetine Dose: adult (70kg+): initially 0.5mg/kg po odfor 7days, maintenace 1.2mg/kg od
(maximum 1.8mg/kg od 120mg od); adult (70kg+): initially 40mg po od for 7 days, maintenance
80-100mg od (maximum 120mg od), may be given as single dose om or in two divided doses with
last dose no later than early evening.
Benperidol Dose: 0.25-1.5mg po od in divided dose; elderly: initially 0.125-0.75mg po od in
divided doses.
Buprenorpfine/Subutex Dose: 200-400mg sl every 6-8h/300-600mg im every 6-8h; initially 0.8-

Bupropion/Zyban Dose: 150mg od for 6 days then 150mg bd for max 9 week (dose if elderly or
seizure risk). Start 1-2 week before target date of stopping smoking; 150mg od, to 300mg if no
improvement after 4 week. NB: max 150mg/day in LF or RF.
Buspirone Hydrochloride Dose: 5mg po bd/tds, at intervals 2/3 days, usual dose 15-30mg od in
divided doses, max 45mg od.
Carbamazepine/Tegretol Dose: initially 100-200mg od/bd (slowly to max of 1.6 g/day or 2 g/day).
Chloral Hydrate/Welldorm Dose: one to two tablets, alternatively 414-828mg po od at bedtime, max
4 tablets/2g od.
Chlordiazepoxide Dose: 10mg tds po, ing if required to max of 100mg/day. NB: dose in RF, LF and
elderly. Dose if benzodiazepine-resistant or in initial Rx of alcohol withdrawal (see reducing
regimen on Alcohol detoxification heading in Alcohol section in Substance Misuse chapter.
Chlormethiazole/Clomethiazole/Heminevrin Dose: 192-384mg po od at bedtime; 192mg po tds;
initially 2-4 192mg capsules, 9-12 capsules in 3-4 divided doses in first 24h, 6-8 capsules od in 3/4
divided doses on day 2, 4-6 capsules od in 3/4 divided doses on day 3, dose gradually over days 4-6,
max duration for 9 days.
Chlorpromazine/Largactil Dose. 25-300mg tds po; 25-50mg tds/qds im (painful, and may BP HR)
NB: dose in elderly (approximately 1/3-1/2 adult dose but 10mg od po may suffice) or if severe RF.
Citalopram/Cipram Dose: 20mg od ing if necessary to max 40mg (max 20mg if elderly og LF)
Clomipramine Dose: initially 10mg po od at bedtime, gradually/alternatively ton30-150mg od in
divided doses, max 250mg od initially 25mg po od, gradually to 100-150mg. Od over 2 week,max
250mg od; start at 10mg po od, gradually to 10-75mg od.
Clonazepam Dose: 1-2mg po od, initially 1mg po od for 4 nights, over 2-4 week, usual dose 4-8mg
od at night, given in three to four divided doses if necessary.
Clonidine Hydrochloride Dose: initially 0.25-0.5mg po od individed dose, by 25mg per week, usual
dose 3-5mg/kg/day; initially 0.5-1mg. Po tds, dose every second/third day, max 1.2mg; od initially
0.5mg po bd for 2 week, to 0.75mg.
Clozapine/Clozaril/Zaponex/Denzapine Dose: initially 12.5mg.nocte, ing to 200-450mg/day usually
given bd (max 900mg/day); slower dose titration if > 60 years. 12.5mg nocte, inc in steps of 12.5mg
up to twice weekly according to response, max 50mg dailiy; in exceptional cicumstances, inc by
12.5mg weekly to max 100mg od in one to two divided doses. If >2 days doses missed, re-titration
necessary. Get specialist advice.
