This spreadsheet is designed to supplement the OPAL Tree Health Survey.
Participants can record information for several trees at one time in the field.

Questions as the questions starting on page 7 of the Field Notebook with the same options. You
will need to use the Field Guide and Most Unwanted guide for images and the Tree Guide to
identify your tree.

Before starting the survey, please read through the survey in full and pay particular attention to
the health and safety information on page 6 of the Field Notebook.

Please return your results to OPAL using either the online submission form or by sending the
form to FREEPOST RSCH-CHYJ-HYYC, OPAL, Centre for Environmental Policy, Imperial
College, South Kensington, London SW7 2AZ. Please also be aware that we may want to
contact you about the results you have provided. By sending the form to us, you are agreeing
