Medical history
: No drug overdose, No drug allergies

: No history of mental illness

: No medical issues
Present medical history:

 Slurred speech, headache and diaphoresis after consumption of 20

tablets of paracetamol after taking 1 bottle of wine.
 Vomited 2 times, drowsy , pale
 Vital signs – BP (100/70) , RR – 12/mt , SPO2- 98%
 Responds to verbal commands ,GCS normal

Nursing management

 Admitted for observation

 Blood tests( AST, ALT, TSH, S. Creatinine, S. paracetamol
concentration
 Chest x-ray. Urine test, Vitals , GCS hourly
 Antidote N acetylcysteine in NS

Patient health status on 2nd December 2018

 Conscious, drowsy, pale

 Vomiting subsided,IV stopped
 Tolerates soft diet and oral liquids
 Shows disinterest in life
 Plan for discharge on 3rd December if the reports are normal

 Review with toxicology

 Referral to psychiatrist for assessment and ongoing care
