Re: Ms. Mary Bells, 66 years old

Q;about the patient’s condition +purpose

I write to refer Ms. Bells who was diagnosed [ADMITTED,PRESENTED ] with infected foot
ulcer and unstable diabetics mellitus. She is progressing well but requires continuity of care
and management as she is getting discharged today.

what happened in the hospital- progression +treatment +medication

During hospitalisation, Ms Bell has shown [showed] good development without any
complications.Additionally, her infected foot ulcer was taken care regularly with silvazine
dressing and was being observed for any untoward [unexpected ,unanticipated ]sign and
symptoms. She was advised to be compliance [policy ] with her diabetic medications as
well as antibiotics (Augmentin) which will be assessed once completed.

· Discharge plan: emphasising the need for an overall life-style plan, and suggesting
involvement of community social worker service.
· Social/family background:=Since retirement alcohol intake has increased and dietary
quality has decreased.
2.Periodic problems with self-administration of hypoglycaemic
medication.

Ms. Bell is a diabetic patient for past 15 years and has periodic problems with self
administration of medications. The patient does not socialize [interact ] with other people and
her alcohol consumption increased significantly along with her poor dietary intake since her
retirement.

In view of the above, it would be greatly appreciated if you could strictly observe Ms Bell's
blood glucose levels, alcohol intake and diet modification. Also, please motivate her to perform
moderate exercises regularly. Finally, kindly collaborate with a social worker to encourage her
to get involved in some social groups [ social activities. ] and income-related activities.[income-
producing activity.]
