Re: Abdulla Gahtani, aged 40

This is to refer Mr Gahtani into your care for further assessment and management due to a
myocardial infraction incident. He is set to be discharged today.

On 7.08.2019, Mr Gahtani was admitted to the hospital with severe chest, jaw and arm pain.
Therefore, a chest X-ray and an ECG were conducted which revealed that he recently had a stroke.
During hospitalization, he underwent CABG surgery and several medications were commenced as
well as ECG monitoring and physiotherapy. Additionally, 150 mg of Karvey and 81 mg of aspirin
were added to his medication. Please note, he does not have a family doctor, is a heavy smoker, and
his BMI is beyond the normal standards.

In view of the above, it would be greatly appreciated if you could provide home visits for Mr Gahtani
for daily blood pressure and heart rate monitoring. Please discuss with him about the benefits of a
healthier diet and smoking cessation. Make sure that he is following his new diet and doing his
regular exercises. Lastly, his next appointment with a cardiologist is on 25.08.2019 at 2 p.m. and
arrange a meeting with a social worker to make sure that his family is registered with the local GP. If
