Re: Maria Ortiz, 7 days old

This is to refer Maria into your care for further observation and assistance. She is set to be
discharged today.

On the 3/04/1997, Maria’s mother, Violetta Ortiz, was admitted to the maternity hospital due to
terminal stage pregnancy being in labour, and on the same day, she gave birth to a healthy baby girl.
During hospitalization, Maria was very relucted to breastfeed and she slept well. When she was
discharged, she gained 20 g.

Regarding Maria’s social history, her parents and her two siblings, Sam and Tessa, live in a rented
two-bedroom flat. Maria’s father, Jose Ortiz, has a job as a security guard doing only night shifts;
therefore, he will not be able to take care of his new daughter which makes Mrs Ortiz very anxious
that she will not cope with the fact that she needs to look after of three children.

In the view of the above, it would be greatly appreciated if you could provide home visits for Maria
and her mother for further help and support in taking care of her. Note that the family do not own a
car, so they will need help with shopping. If you require further information, please do not hesitate
to contact me.
