Source Cobalt60 Re-Export Project, Gatot Army Hospital as below:

A. Scope of Work:
1. Importing radioactive container from Canada to Jakarta, Indonesia.
2. Custom clearance and transportation for radioactive container from Soekarno-
Hatta International Airport to Gatot Soebroto Army Hospital.
3. Business visa for foreign expert engineers to Indonesia.
4. Unloading Cobalt-60 source from Teletherapy unit to radioactive container.
5. Disassembling of Cobalt-60 Teletherapy Unit.
6. Wipe Test from BATAN.
7. BAPETEN permit and license as detailed:
 Activation of expired Operational License from BAPETEN for Cobalt-60
Teletherapy unit at Gatot Soebroto Army Hospital.
 Import license of Cobalt-60 source container
 Import permit of Cobalt-60 source container
 Temporary Import license for Decommisioning tool
 Transport permit of Cobalt-60 source container from Soekarno-Hatta
International Airport to Gatot Soebroto Army Hospital
 Export license of depleted Cobalt-60 source
 Export permit of depleted Cobalt-60 source
 Apply Termination Operational Permit
8. Transporting depleted Cobalt-60 source from Gatot Soebroto Army Hospital to
Soekarno-Hatta International Airport.
9. Reporting Radioactive Material Receiving Report to complete the hospital
Operational License.
10. Disposal of Cobalt60 Source at Country of Origin
