1. Explain the role of the school's OBT Office in the aspect of monitoring
and reporting of the cadets undergoing OBT Program(15 pts);
The OBT Office monitors the progress of students undergoing OBT by:
I. Communicating, checking and providing guidance as to the
accomplishment of TRB;
II. Maintaining records of monitoring;
The record shall include, among other:
i. Student’s Name
ii. Program Enrolled
iii. Date of Enrollment
iv. Contract/ Standard Cadet Training Agreement
v. Date of Embarkation
vi. Date of disembarkation
vii. Route (domestic or International)
viii. Ship’s Particular
ix. Onboard Welfare (complaints / concerns)
The OBT Office shall be responsible for the management of the activities and
other requirements for students undergoing the OBT. Thus, for the
maintenance and operation of this Office, the following shall be established:
A. a fulltime OBT Supervisor and sufficient staff to effectively manage the
daily operation of this Office;
B. a separate workplace with an area of at least 20 sq. m.;
C. resources and facilities to support its effective and efficient operation,
such as but not limited to:
1. at least one (1) set of computer unit with internet connection and a
fax machine or (an equipment and related peripherals which serves
the purpose of sending and receiving documents);
2. tables and chairs;
3. at least one (1) telephone line; and
4. a bulletin board;

2. Explain the first compliance of the OBT cadets' submission of document


for the OBT Office right after their embarkation(15 pts); and
The Onboard Training Monitoring and Reporting is carried out through Social
Media, such as Facebook. Email or Facebook Messenger Every three (3) months
following CCI (Completed Classroom Instructions), students should report to
the School (MHEI) via Facebook Messenger or Email about their status.
Students/Cadets on Onboard Training must complete the Placement and
Shipboard Monitoring Form by FB Messenger or Email within one month of
embarkation. This is necessary to better advise the students/cadets and
document the current status of their deployment.

3. Discuss the requirements for the issuance of Endorsement /


Recommendation Letter by the Onboard Training Office for students
who wish to undergo Onboard Training to various manning/shipping
agencies(20 pts).
For those students who wish to undergo Onboard Training to various
manning/shipping agencies that requests for an Endorsement/Recommendation
Letter, an Endorsement/Recommendation Letter will be issued by the Onboard
Training Supervisor to help them with their application to the respective
manning/shipping agencies, with requirements below.
Requirements For Issuance Of Endorsement/Recommendation Letter at the
Onboard Training Office:
I. Transcript of Record (TOR)
a list of the course units taken, the exams you have passed and the
credits gained.
II. Diploma
a certificate awarded by an educational establishment to show that
someone has successfully completed a course of study.
III. Certificate of Good Moral Character
a certificate having a personal history that demonstrates honesty,
fairness, and respect for the rights of others and for the law.

IV. Passport
a formal document issued by an authorized official of a country to one
of its citizens that is usually necessary for exit from and reentry into
the country
V. Seaman’s Book
a comprehensive document in which several details and aspects of the
seaman’s professional life are stored.
VI. NBI Clearance
government document issued to a person upon request which certifies
that the person concerned is not involved in any pending criminal
proceedings in the Philippines at the time of the submission.
VII. Seaman’s Registration Certificate (SRC)
document that states your current capacity or the type of job you can
perform on board a vessel.
VIII. Training Certificates
a document certifying that the holder has taken a specific course of
training.
