AFFIDAVIT OF LOSS

I, SUSAN ALOG GARCIA, of legal age, married, Filipino Citizen and a

resident of Brgy. Balabag, Dumangas, Iloilo, Philippines, after having been
duly sworn to in accordance with law do hereby depose and state:

1. That I am a holder of a Philippine Passport duly issued by the office of

the Department Affairs (DFA) Iloilo;

2. That said Passport was kept inside my bag for safekeeping reasons;

3. That however at present I can no longer find the said passport and
although I had tried to look for the said Passport the same can no
longer to be found;

4. That despite diligent search and efforts to locate the said Passport, I
could not find the same such that I believe that it is now totally lost
and beyond recovery; 

5. That I am executing this affidavit to attest to the foregoing facts of loss

of my passport, in order to support my application for the issuance of
a new passport in lieu of the one which was lost, and also in order to
apply for the renewal of my passport with the office of the DFA ILOILO,
and for whatever legal purpose this may serve best.

and a resident of Brgy. Tabucan, Dumangas, Iloilo, Philippines, after having
been duly sworn to in accordance with law do hereby depose and state:

6. That I am a holder of a Philippine Passport duly issued by the office of

the Department Affairs (DFA) Iloilo issued on September 18, 2010 and
which expired on September 17, 2015;

7. That said Passport was kept inside my bag for safekeeping reasons;

8. That however at present I can no longer find the said passport and
although I had tried to look for the said Passport the same can no
longer to be found;

9. That despite diligent search and efforts to locate the said Passport, I
could not find the same such that I believe that it is now totally lost
and beyond recovery; 

10. That I am executing this affidavit to attest to the foregoing facts

of loss of my passport, in order to support my application for the
issuance of a new passport in lieu of the one which was lost, and also
in order to apply for the renewal of my passport with the office of the
DFA ILOILO, and for whatever legal purpose this may serve best.

AFFIDAVIT OF LOSS

I, ROSE DOCTORA MACADAG-UM, of legal age, married, Filipino

Citizen and a resident of Brgy. Lagubang, Barotac Nuevo, Province of Iloilo,
Philippines, after having been duly sworn to in accordance with law do
hereby depose and state:

11. That sometime in the year 2013 I was issued a Philippine

Passport by the Department of Foreign Affairs (DFA) in Iloilo City
which would expire in September 2018;

12. That I used to place the said Passport inside a plastic envelope
together with some of my important documents for safekeeping;

13. That on September 29, 2017 I went to Iloilo City and made some
important transactions which I brought with me the said plastic
envelope that contains my passport;

14. That when I came home in the evening of the aforementioned

date, I found out that my passport was no longer inside the plastic
envelope and it was believed that the said passport slipped out from
the white envelope without my notice when I alighted from the public
utility vehicle (PUV) while I was in Iloilo City;

15. That despite diligent search and efforts to locate the said
Passport, I could not find the same such that I believe that it is now
considered as totally lost and beyond recovery; 

16. That the said loss was reported to the Barotac Nuevo Police
Station as evidenced by a Certification of the Police Blotter Report
dated October 23, 2017, herein attached and marked as Annex “A”;

17. That I am executing this affidavit to attest to the foregoing facts

of the loss of my Passport and to support the application for the
issuance of a new passport in lieu of the one which was lost.
