Greetings.
The undersigned would like to request your attention regarding the herein
photocopied check issued to GARY DENNIS I. VILLALUZ on October 17, 2012
with Check No. 0000530916 in the amount of Thirty-nine Thousand Nine
Hundred-two Pesos and 44/100 (P39,902.44). This was allowed to be encashed
even without my signature as one of the official signatories on the said check. To
my dismay, the said check was only given to me on October 30, 2012 for
signature – days after the said check was encashed.
I am bringing this matter to your end for proper action that it may not be
happened again. The concerned employee specially the verification section and
the tellers should have to be careful and be responsible in all the things they do
while performing their duties and functions.
Hoping that this matter will be given due attention and proper action will be
instituted.

Please be informed that Check No. 0000530916 issued to HON. GARY DENNIS
I. VILLALUZ on October 17, 2012 in the amount of Thirty-nine Thousand Nine
Hundred-two Pesos and 44/100 (P39,902.44) was encashed even without my
signature as one of the official signatories on the said check and was only given
to me on October 30, 2012 for signature by one of your employees – days after
the said check was encashed. Photocopy of the said check is hereto attached.

Relative to this, the undersigned would like to request your good office to kindly
remind your assigned employee to be responsible in doing their duties and
function and pay respect to all official signatories particularly in the check by
making it sure that all of them have affixed their signature before it will be
encashed.

Please be reminded that this kind of practice is violating the Code of Ethics of the
Public Officials and Employees and punishable by law.
