Subject: Explanation for quality & quantity monitoring failure of Operations DEPT.

Dear Board,

Please accept this letter of explanation in regards to our failure in monitoring operations team quality
and quantity. We failed in the same due to lack of diligence from the team on submitting the audit
reports on time and we didn’t escalate the same to management on time which is leading to
inappropriate methodology of monitoring the quality of operations processes.

To improve audit methodology, we have recently automated few of our operations process audit report
to reduce manual errors.

Going forward we will live up to excellence.
