MEMORANDUM OF AGREEMENT
S.Y 2020- 2021

This memorandum of agreement is made and entered into in the Province of Pangasinan, Municipality
of Alcala, Barangay Canarvacanan, Alcala, Pangasinan.

Parent-Teacher Association of Canarvacanan National High School by its school head, Engr. Armando
P. Olpindo.

WHEREAS, Canarvacanan National High School is receiving additional fund for Senior High School
of Php 117,000 through the school head for the payment of utilities and purchase of school and office
supplies, medical supplies, materials for repair and all other school needs pursuant to Deped Order No.
8, s. 2019, Deped Order No. 12, s. 2014, Deped Joint Circular o. 2004-1 dated January 1, 2004 and all
other legal basis to this effect.

WHEREAS, the said payments of utilities and purchase of school and office supplies, medical
supplies, materials for repair and all other school needs will not be charged against any PTA fund. All
aforementioned purchases and payments will be done utilizing the monthly cash advances.

WHEREAS, whenever there are trust funds made available for the school such can also be utilized for
the payment of utilities and purchase of all school needs such as office supplies, medical supplies, and
construction materials for repair or as may be provided by governing laws of RA 9155 (Governing
Basic Education Act 2001).

NOW, therefore, for and in consideration of the foregoing premises, Canarvacanan National High
School PARENT-TEACHERASSOCIATION AND CANARVACANAN NATIONAL HIGH
SCHOOL hereby agree on this agreement and that copies of the MOA be made available to the office
of the PTA Secretary, PTA President, Mrs. And the office of the school head, Engr. Armando P.
Olpindo.

We hereby certify to the correctness of the foregoing Memorandum of Agreement
