persons of the First Judicial District of Hinds County, in the State of Mississippi, elected,
impanelled, sworn and charged to inquire in and for said District, County and State aforesaid, in
the name and by the authority of the State of Mississippi, upon their oath present: That

In said District, County and State

COUNT I

on or about the 1st day of November 2021, in the county aforesaid and within the
jurisdiction of this Court, the said defendant being a female human being above the age of
eighteen ( 18) years, whose date of birth is September 12, 1970, did willfully, unlawfully, and
feloniously, while operating a motor vehicle in violation of Section 63-11-30 (I)( d)(i) and
operating such vehicle in a negligent manner caused the death of Allison Conaway, a female
human being, to-wit: Beth Ann White, while having a blood alcohol concentration of 0.273% did
operate a motor vehicle, a 2011 Aucura TL automobile, in a negligent manner causing an
automobile accident with a 2017 Nissan Maxima automobile, which automobile was being
driven by Allison Conaway, which automobile accident caused the death of Allison Conway, all
occurring within the jurisdiction of this court and in violation of Section §63-11-30(5)(a)
Mississippi Code Annotated ( 1972, as amended);

COUNT II

based upon a series of acts connected together and constituting parts of a common
scheme and plan, said Beth Ann White being a female human being above the age of eighteen
( 18) years, whose date of birth is September 12, 1970, did, on or about the 1'1 day of November
2021, in the county aforesaid and within the jurisdiction of this Court, willfully, unlawfully, and
feloniously, while operating a motor vehicle in violation of Section 63-11-30 (1 )( d)(i) and
operating such vehicle in a negligent manner caused the death of Alex Conaway, a male human
being, to-wit: Beth Ann White, while having a blood alcohol concentration of 0.273% did
operate a motor vehicle, a 2011 Aucura TL automobile, in a negligent manner causing an
automobile accident with a 2017 Nissan Maxima automobile, which automobile was being
driven by Allison Conway and in which automobile Alex Conaway was a passenger, which
automobile accident caused the death of Alex Conway all occurring within the jurisdiction of
this court and in violation of Section §63-11-30(5)(a) Mississippi Code Annotated (1972, as
amended);

COUNT III

based upon a series of acts connected together and constituting parts of a common
scheme and plan, said Beth Ann White being a female human being above the age of eighteen
( 18) years, whose date of birth is September 12, 1970, did, on or about the I st day of November
2021, in the county aforesaid and within the jurisdiction of this Court, willfully, unlawfully, and
feloniously, while operating a motor vehicle in violation of Section 63-11-30 (1 )( d)(i) and
operating such vehicle in a negligent manner caused the mutilation, disfigurement, permanently
disabled or destroyed the tongue, eye, lip, nose or any other limb, organ or member of the body
of Chloe Conaway, a female human being, to-wit: Beth Ann White, while having a blood alcohol
concentration of 0.273% did operate a motor vehicle, a 2011 Aucura TL automobile, in a
negligent manner causing an automobile accident with a 2017 Nissan Maxima automobile,
which automobile was being driven by Allison Conway and in which automobile Chloe
Conaway was a passenger, which automobile accident caused the mutilation, disfigurement,
permanently disabled or destroyed the tongue, eye, lip, nose or any other limb, organ or member
of the body of Chloe Conway , all occurring within the jurisdiction of this court and in violation
of Section §63-11-30(5)(a) Mississippi Code Annotated (1972, as amended);

COUNT IV

based upon a series of acts connected together and constituting parts of a common
scheme and plan, said Beth Ann White being a female human being above the age of eighteen
(18) years, whose date of birth is September 12, 1970, did, on or about the I'' day of November
2021, in the county aforesaid and within the jurisdiction of this Court, willfully, unlawfully, and
feloniously, while operating a motor vehicle in violation of Section 63-11-30 (1 )( d)(i) and
operating such vehicle in a negligent manner caused the mutilation, disfigurement, permanently
disabled or destroyed the tongue, eye, lip, nose or any other limb, organ or member of the body
of Chelsea Conaway, a female human being, to-wit: Beth Ann White, while having a blood
alcohol concentration of0.273% did operate a motor vehicle, a 2011 Aucura TL automobile, in a
negligent manner causing an automobile accident with a 2017 Nissan Maxima automobile,
which automobile was being driven by Allison Conway and in which automobile Chelsea
Conaway was a passenger, which automobile accident caused the mutilation, disfigurement,
permanently disabled or destroyed the tongue, eye, lip, nose or any other limb, organ or member
of the body of Chloe Conway , all occurring within the jurisdiction of this court and in violation
of Section §63-11-30(5)(a) Mississippi Code Annotated (1972, as amended);

All of the above, Count I, Count II, Count III and Count IV being contrary to the form of the
statute in such cases made and provided, and against the peace and dignity of the State of
Mississippi.
