ATP - EMTP ORDER FORM

The following ATP-EMTP materials can be obtained from Dr. Kai-Hwa Ger, ATP Distribution/Licensing
Coordinator, in West Linn, Oregon, USA. Receipt and usage of these materials require ATP licensing using the
associated form letter having date of original writing 30 January 1992. If you have not signed this licensing
agreement yet, fill in the blanks and return a signed copy to Dr. Ger along with your ATP-EMTP order form. All
prices are in American dollars (abbreviated "US$"), and subject to change by Dr. Ger without notice. One must
enclose a check (drawn on an American bank) or international money order payable in U.S. currency. Details of
this order are important: 1) The user group will not respond to any purchase order that attempts to qualify or
modify any condition of the transaction (often in small print on the reverse side); 2) The user group will not bill the
ordering party; 3) the check must not be made payable to the user group (no bank account exists in this name, so
any such check will be returned); 4) the user group will not provide a receipt, although it will sign and return one
that has been prepared by the user. 5) As stated in the April, 1996, newsletter, the check or money order must have
magnetic encoding to be processed free of charge by an American bank.
