QUITCLAIM AND RELEASE

KNOW ALL MEN BY THESE PRESENTS:

That I, __________________________, of legal age, Filipino and with  address at

_______________________________, Solana, Cagayan, in consideration of 
__________________________________________ (P__________), received by me in
full and to my entire satisfaction for all moneys and benefits, specifically,
UNDERPAYMENT OF WAGES, NON-PAYMENT OF OVERTIME PAY, AND NON-
PAYMENT OF SERVICE INCENTIVE LEAVE PAY, due me in connection with my
present employment at TREASURE MARKETING and/or JEROME LI, hereby release
and forever discharge TREASURE MARKETING and/or JEROME LI including all its
officers, from any and all money claims, causes of action  or otherwise.

That I have no more claims or rights of action of whatever kind and nature against the
said Company and any officers thereof.

That I hereby confirm that this Release and Quitclaim is being executed by me
voluntarily and in a language familiar to me and is not intended to circumvent or defeat
any provision of law, rule or regulation.

That I hereby attest that I fully understood all the terms and conditions of this Release
and Quitclaim as well as the legal implications thereof and I hereby express my
complete agreement thereto.

IN WITNESS THEREOF, I hereunto affix my signature this _____ of June 2022 in the
