Learning History Acknowledgement

I confirm I have received and understand the Learning History process updates that include the
following changes:

 If incorrect credit details were provided on a learning history record, an LHMR request much be
submitted and assigned to your appropriate Team Lead for processing. The details of the LHMR
ID should be documented in the Log of the LHIST request for reference. This update is tied to
QER0085 CAPA #1.
 When processing a learning history that contains an instructor, after associating the instructor
to the learning history, you are now required to copy the instructor ID from the “Instructor ID”
field and place that information in the “Comments for Admin” field in the following format:
Instructor ID: (insert GID of instructor)
