AFFIDAVIT OF NON-OPERATION

I, ALASTAIR SOLIANO CAÑETE, of legal age, single, Filipino and with the
residence address at Datag Baybayon, Maribago, PH-6015, Lapu-Lapu City, Cebu,
Philippines after having been duly sworn to in accordance with law, hereby depose and state:
1. That I am the owner of KUYA A TOUR SERVICES with business address at
Datag Baybayon, Maribago, PH-6015, Lapu-Lapu City, Cebu;
2. That the said business was registered under the Department of Trade and Industry
(DTI) at Lapu-Lapu City, Cebu on the 15th day of November 2018 with Certificate
No. 05626974;
3. That the aforesaid business has temporarily stop its operation and has not conduct
any transaction of its business from March 26, 2020 until the present date due to
the outbreak brought by COVID-19 pandemic and typhoon Odette;
4. to the outbreak brought by COVID-19 pandemic and typhoon Odette;
5. As such, I am executing this affidavit to attest to the truth of the foregoing and for
the purpose of complying with the reportorial requirements of the local
government unit of Lapu-Lapu City and other government entities.
