THE HOSPITAL TEAM

A big hospital is like a small town, it needs a thousand of people to make it work.

All these people are organized in to teams and each person in each team has a rank and

often a specialism. In the past, the health care team’s chain of command was simple:

doctors made decisions and gave senior nurses orders. Senior nurses then instructed junior

nurses, and so on. Things are changing. In many countries nurses have much more

responsibility than they are had.

One thing is not changing it is still a doctor who is in charge of a patients

treatment. In Britain that doctor is called a consultant. Next in line to the next consultant

is the registrar. Consultant and registrar train the junior doctor who are called house

officer or in terms. Junior doctor work the same long, unsocial or that nurses do on the

wards. The most senior nurses are nursing officer they are administrator. Than come war

manager who supervise staff nurses and student.

It is not only doctor and nurses who deliver treatment and care, there are many

other specialist too. For example there are pshyoteraphist who specialist exercises to treat

injury or dysfunction and occupational therapist who help patient manage everyday

living. There are also many people the patient does not meet like lab technician doing the

test and pharmacist dispensing medicines

2 HOSPITAL ADMISSION

HOSPITAL ADMISSION

There are three ways to admit a patient a patient to hospital. Either as an


outpatient, a day patient or an inpatient . an outpatient need treatment but not a bed. A
day patient need a bad but does not need to stay overnight. An inpatient need at least one
over night stay. Hospital need to plan for patient admissions. They have to predict how
many bed they need. It is quite easy to predict referral from doctor like general
practitioner but a larger number of patients come through A & E and it is more difficult
to plan for them.
It is very important to keep accurate record of patient treatment, from admission
through to discharge and follow up. These include personal details such us marital
status, occupation and next of kin, as well as medical history such us past illnesses and
treatments, family history and life style. Medical staff record all treatment, test result
and correspondences, they try to record these thing at the time they happen. This is
important and not only to ensure the correct treatment. Medical record are sometimes
used for research.

PAIN

Pain is important because it tells us that we are injured or ill. However, we don’t all
feel pain in the same way. Researchers are trying to learn more about this fact. Their
experiments show that children are more sensitive to pain than adults, and that men can
tolerate more pain than women.

Pain is also difficult to measure and describe. This is a problem because it is an


important symptom and medics (medical staff) need information from patients about it. It is
therefore common practice to give patients lists of words and ask them to say which words
best describe three things: the type of pain they are suffering, its intensity (how bad it is )
and its frequency (how often they feel it).

With some patients, such as children, words don’t work very well describe intensity,
so medics use smiley feces or sometimes colors. For example, blue mean a mild pain and
reds mean severe pain. Some medics prefer a range of numbers; 0 is no pain and 10 is
unbearable pain.

Pain does not always show where an injury is. Internal organs, for example, do not
have many pain-receiving nerve endings, so internal injuries often cause pain in a different
part the body. This is called ‘referred pain’. One example of referred pain is when someone
suffering a heart attack feels pain in their left shoulder, arm or hand.

SYMPTOMS

Signs and symptoms are the tools for making a diagnosis, but what is the difference
between the two ? The sings of an illness are the things that a doctor or nurse can see and
measure. Signs are things like spots and bleeding. Temperature, heart rate, blood pressure
and respiration rate are all signs because you can measure them. Symptoms are the things
which a patient experiences, but others can’t always see. Dizziness and nausea are
examples of symptoms.

The symptoms which make a patient seek medical help in the first place are called
the presenting symptoms. Medics describe them in terms of being either strong, mild or
weak. Sometimes the symptoms of serious illnesses like cancer and diabetes are weak.
They stay weak for a long time and the illness remains undiagnosed.

Many illnesses have the same symptoms. These symptoms are non-specific. Fatigue
in an example of this. It’s a symptom of many kinds of illness, both chronic and acute, and
of both physical and mental disorders.
Doctors ask patients about the onset of the symptoms, what they feel like, what
relieves them and what makes them worse. The more detail they have, the faster they can
make a diagnosis.

NUTRITION AND OBESITY

Nutritionists classify food by groups. They say that the humans body requires a
balanced mixture of food from all the main food groups: carbohydrates, protein, fast,
vitamins, fibred and minerals. The secret of good heath is to eat a balanced diet, with the
right amount of food, and to take plenty of physical exercise.

When there is an imbalance-for example if you eat too little or too much, or when
one of the food groups is missing from your diet-this can cause vitamin deficiency, illness,
and even death. If there is no treatment and the diet does not change, death is a possibility.
One example of this is the problem of obesity-this is when a person is dangerously
overweight. Obesity is mist common in developed countries. The imbalance comes from
eating a diet which is high in fat, sugar and salt, and in to not eating enough fruit, vegetables
and fibred, and not burning the extra calories through physical exercise.

it the case of eating disorders such as anorexia nervosa and bulimia, the imbalance
leads to weight loss, when a person eats less than they need. Sufferers are mostly teenage
girls- they either starve themselves in order to lose weight, or eat a lot of food at once
(binge-eating) and vomit up all the food later. They often end up in hospital, and after
emergency life-saving treatment they receive psychiatric help and counseling.

BLOOD
The prefix haem-means blood the study of blood is hematology, hemophilia is a disease of
the blood and a haemorhage is a bruise. In humans, blood forms in the bone marrow by a process
called haemogenesis. Blood circulates around the body through the cardiovascular system of
arteries, veins and capillaries.
Blood has many functions. It distributes oxygen, keeps the body warm, and remover waste.
More than half of the blood in the body is a fluid called plasma-plasma is mostly water. The rest of
the blood is made up of red cells which carry oxygen, white cells which protect against disease, and
platelets which heal wounds.
When someone loses a lot of blood they need transfusion. There are different types of
human blood and medics usually use the ABO system which puts blood into four main groups:
A,B,AB,and O. The donor blood must be compatible with the blood of the recipient, because a
transfusion of the wrong blood can be fatal.
Medical conditions affect blood in many ways, so a blood test is a good way to get
information about a patient’s state of health. The shape, size, behavior and number of blood cells can
provide information about kidney and liver function, about levels of hormone, glucose and
cholesterol, and about a patient’s immune system.

Cervical Cancer

The cervix is the lower part of the uterus, the place where a baby grows during

pregnancy. Cervical cancer is caused by several types of a virus called human

papillomaviruses (HPV). The virus spreads through sexual contact. Most women's bodies

are able to fight HPV infection. But sometimes the virus leads to cancer. You're at higher

risk if you smoke, have many children, use birth control pills for a long time, or have HIV

infection.

Cervical cancer may not cause any symptoms at first, but later, you may have pelvic

pain or bleeding from the vagina. It usually takes several years for normal cells in the cervix

to turn into cancer cells. Your health care provider can find abnormal cells by doing a Pap

test - examining cells from the cervix under a microscope. By getting regular Pap tests and

Responses to Health and Illness

Irish immigrants have high rates of mental illness. Although children of Irish

immigrants have fewer psychological problems than their parents, they lead all other

second-generation Americans in frequency of mental health problems (Blessing, 1980).

One explanation for high rates of mental illness may be associated with the Irish

having difficulty describing emotions and expressing feelings. The health-care provider can

encourage the expression of emotions and feelings before symptoms become a problem.

In the past, the mentally and physically ill were taken care of in the home, not

“Gender Differences in Health”

“Only in recent years we have learned much about health-related differences

between men and women, largely because up until the 1990s women were systematically

excluded from medical research (Hamilton, 1996).

The rationale for exclusion of women from clinical investigations was that women’s

reproductive functions (e.g., pregnancy, menstruation, hormonal changes) might interfere

with the study protocol and distort research findings.

The creation of the national Office of Research in Women’s Health in 1991 led to

legislation mandating the inclusion of women and minorities in all federally funded

research. Consequently, we are learning more and more about gender differences in

physiology, metabolism, response to medication, organ function, and the progression of

disease.”.

Considering the population explosion, increasing inflation and scarcity of resources,


the family trends have shifted to nuclear families with number of children limited to one or
two. Family planning refers to the natural and artificial birth control methods that allow you
to control the size of your family and the gap between your children.
Family planning helps you prevent conception and reduce the risk of pregnancy. The
biggest advantage of family planning tools is that they save you from the hassles of
pregnancy termination and abortion. Adopting a family plan directly effects a woman's

Doctor : How long have you been having this pain?


Patient : It started in June. So for more than five months now. My stomach hurts
after some meals, but not always.
Doctor : You should have come in earlier. Let's get to the bottom of this. Have you
changed your eating habits during this period?
Patient : No, not really. Well, that's not true. I'm eating the same foods, but less.
You know, the pain seems to come and go.

A Cesarean Section

A Cesarean section (C-section) is surgery to deliver a baby. The baby is taken out through
the mother's abdomen. In the United States, about one in four women have their babies this
way. Most C-sections are done when unexpected problem shappen during delivery. These
include Health problems in the mother The position of the baby Not enough room for the
baby to go through the vagina Signs of distress in the baby C-sections are also more
common among women carrying more than one baby.

The surgery is relatively safe for mother and baby. Still, it is major surgery and carries
risks. It also takes longer to recover from a C-section than from vaginal birth. After healing,
the incision may leave a weak spot in the wall of the uterus. This could cause problems with
an attempted vaginal birth later. However, more than half of women who have a C-section
can give vaginal birth later.
