AFFIDAVIT OF UNDERTAKING

I, EDGAR P. LAGAHIT, Filipino, of legal age, married and a resident of Purok 2, South
Poblacion, Maramag Bukidnon after having been sworn to an oath in accordance with law,
hereby depose and say:

1. That I am a retired employee of Radio Communications Office Provincial Governors

Office of Bukidnon;

2. That I retired as Administrative Aide VI last July 1, 2019;

3. That the Local Government Unit of the province of Bukidnon is granting an Awards and
Incentives for Service Excellence (PRAISE);

4. That I am one of the grantee of the said award and incentive mentioned above;

5. That in case the said award will be invalidated, I undertake to refund or return the same;

6. That I am executing this affidavit to attests to the truth of the foregoing and for any legal
purpose it may serve.
