PETITIONER, xxxxxx most respectfully files this

application for appointment and commission as a Notary
Public for and in the City of Cebu, for a term commencing on 1
January 2019 and ending on 31 December 2020 and avers
that:  
1. Petitioner possesses the eligibility for commissioning
as Notary Public as shown by the following personal

2. Petitioner has satisfactorily completed the required

Bar Examinations and admitted to the practice of law on June
14, 2019. Attached herewith is a copy of the Certificate of
Membership in the Philippine Bar as Annex “A”, and a copy of
the Professional Tax Receipt due her is hereto attached as
Annex “B”;

3. Petitioner holds office at the following address:

4. Petitioner is a resident and natural born citizen of the

Philippines since birth;

5. Petitioner has not been convicted, much less, charged

with any offense involving moral turpitude or any criminal or
civil offenses for that matter. Attached herewith as Annex “C”
is the NBI Clearance issued by the National Bureau of
Investigation;

6. Petitioner is a member of good standing of the

Philippine Bar as shown by the following certification:

a. Certification of No Pending Administrative Case

issued by the Bar Confidant’s Office of the Supreme
Court, Manila is hereto attached as Annex “D”,

b. Certification of No Criminal Case filed against the

Petitioner issued by the Office of the Clerk of Court,
Regional Trial Court and Municipal Trial Court in
Cities, 7th Judicial Region, Cebu City is hereto
attached as Annex “E” and Annex “E-1”,
respectively.

c. Joint Certification as a Member of Good Standing

and of Good Moral Character issued by IBP-Cebu
Chapter is hereto attached as Annex “F” and a
copy of the Official Receipt of Membership by the
Integrated Bar of the Philippines is hereto attached
as Annex “G”;

7. Petitioner now desires to be appointed and

commissioned as Notary Public acting for and in the City of
Cebu, for the term ending on 31 December 2020;

 8. Three (3) of Petitioner’s passport-size colored
photographs with name and signature at the bottom thereof
are attached in this petition as Annex “H” and a separate
sheet of paper forms part of this petition containing three (3)
specimens of petitioner’s signature is likewise attached as
Annex “I”;

9. Petitioner has not applied for nor was she issued a

Commission as a Notary Public in any other jurisdiction for
the current year;

10. Petitioner has paid in full the proper filing fees;

11. Petitioner undertakes to submit or report to the Office

of the Clerk of Court copies of all documents and deeds
notarized by her.

12. Petitioner possesses all the qualifications and none of

the disqualifications and desires to be appointed as a Notary
Public for the City of Cebu and within the jurisdiction of this
Honorable Court.

PRAYER

WHEREFORE, Petitioner most respectfully prays that she

be appointed and commissioned as Notary Public for and in
the City of Cebu and within the territorial jurisdiction of this
Honorable Court. It is likewise prayed that Petitioner be
authorized to purchase her notarial seal for the purpose.
