Mrs. C, 27 y.o., G2P1, came to your labor ward at 8 a.m. on Nov 25th 2012 with chief complaint of having contractions
since 8 hours before admission, and feeling leakage of amniotic fluid since 2 hours before admission. On examination,
there were 1-2 contractions within 10 minutes, with 20 seconds duration each. Head descent 4/5, fetal heart beat was
140x/minute, blood pressure was 110/80, with body temperature of 36,8 0C and heart rate was 90x/minute. The clear
amniotic fluid came out from vagina. The portio was anterior, soft, cervical dilation was 3 cm, 1 cm in thickness, no
amniotic membrane, no moulage. No edema and the last Hb level was 11,4 g/dl. No proteinuria or aseton found from
laboratory examination. You have decided to give oxytocin 5 IU in 500 mL ringer lactate and starting from 4
mIU/minute to accelerate the delivery process. Adequate contraction was achieved by oxytocin 6 mIU/minute.

At 10.30 a.m, the mother complaint of more

painfull contractions. On examination, there were 3-4 contractions within 10 minutes, with 50 seconds duration each.
Head descent 3/5, fetal heart beat was 148x/minute, blood pressure was 110/80, with body temperature of 37 0C and
heart rate was 90x/minute. The portio was anterior, soft, cervical dilation was 6 cm, no moulage, head on H I – II, clear
amniotic fluid.

From observation of fetal heart beats, contractions, and maternal heart rates every 30 minutes the results below was
obtained:

At 13.00 the mother had feel to bear down. From examination, there were 4 contractions within 10 minutes, each last
for 50 seconds. Fetal heart beat was 146x/minute, head descent 1/5. From vaginal examination, the cervix was
already fully dilated, no amniotic membrane so that the baby’s hair can be palpated, and no moulage.

The mother was conducted to bear down. Five minutes later a female baby was spontaneously born, with birth weight
of 3300 gram and baby’s length was 50 cm, Apgar score was 9/10. Third phase active management was performed,
and the placenta was expelled completely 5 minutes after the delivery. The perineum was intact. Total bleeding was
+100 cc.
