Request for different work schedule/arrangement of employees with immunodeficiency, comorbidities or other health risks/conditions must be supported with medical certificate and other medical records.” Further, Section 13 provides that “Women have the right to protection and security in times of disasters, calamities and other crisis situations....xx". ry 2, Along this line, all concerned school heads and section/unit heads of this Office are Tequired to identify the employees within their jurisdiction who belong to those enumerated above and provide a preferable five-day work from home arrangement schedule, These ‘employees are required to submit the following: Medical certificate and other medical records, b. Affirmation Letter signed by the Distric/Office Nurse; ©. Weekly Plan of Activities/Tasks, and 4. Weekly Accomplishment Report 3. The enclosed “Weekly Plan of Activities/Tasks and Weekly Accomplishment Report templates must be followed. 4, The concemed school heads and section/unit heads are required to strictly monitor the daily attendance of these employees via appropriate online mechanisms (google form ete..), 10 ensure that the weekly plan of activities/ tasks are done.
