AFFIDAVIT OF UNDERTAKING FOR AUTHORITY TO DEDUCT

ACCOUNTABILITIES

I, EVANGELINE A. ALMONDIA, 65 years old, Filipino, married and a resident

of Brgy. Capirpiriwan, Cordon, Isabela, personally appeared before the undersigned
notary public, and under oath hereby depose and say:

1. That I am the one and the same person executing this Affidavit;

2. That I am at present a Department Ed TEACHER III at Capirpiriwan Elementary

School, Cordon, Isabela;

3. That I am an active member of the GSIS and PAG-IBIG;

4. That I have been teaching for FOURTY (40) years;

5. That I will avail my retirement option under R.A. 8291;

6. That I am now authorizing the proper authorities to deduct from my cash

receivables any accountabilities/loan that I might have incurred during the course of my
employment;

7. Further, I am executing this Affidavit to declare the truth of the foregoing facts
voluntarily, and without defrauding the duly constituted authorities or whatsoever, the
purpose herein intended for.
