3753, and other Laws on Civil Registration, the


Local Civil Registry Office receives and keeps records showing the different facts of the
beginning and end of the civil personality and modification in a person.  These facts are: 1) Birth
2) Recognition 3) Legitimation 4) Adoption 5) Marriage 6) Legal Separation, Annulment/Voiding
of Marriage 7) Death 8) Election of Philippine Citizenship, Naturalization and Repatriation 9)
Other Court Decrees and Legal Instruments affecting the civil personality of a person.

ISSUANCE OF REPRODUCTIONS/TRUE COPIES OF CIVIL REGISTRY RECORDS

Certified true copies of Civil Registry records are issued upon written request.
Normally, this transaction can be completed in two (2) working days.

Who May Avail of Service: (for requests for Birth records)

1. Parents
2. Person Concerned/Owner
3. Authorized Representative
4. Courts and other Public Offices

Requirements:

1. Accomplished request form from Local Civil Registrar Office counter.


2. Any valid ID with picture and signature such as driver’s license, school, office,
GSIS, and SSS.
3. Authorization letter for person representing owner.
4. Letter from a public office requesting copy for administrative purposes.

Fees:
Ordinary/Local use – P 30.00 per copy
Travel abroad – P 75.00 per copy
Certified machine reproduction – P 30.00 per copy
Other certifications – P 30.00 per copy
