II. CASE STUDY

Community Nurse Mara conducts home visitation at Brgy. Portero where she encounters Family XYZ whose house is built beside a river. While
conducting the home visit, Nurse Mara obtained information regarding the family’s health status. Family XYZ is composed of 12 members, the
mother who at present is 3 months pregnant with no prenatal care. The father is working as a “mangangalakal” that serves as the family’s primary
source of income where the father is able to earn 100 pesos per day with a total of 3,000 pesos in a month. During visitation the nurse observed that 8
month baby is bottle fed using water directly from the faucet. Amongst the children ages 3, 5, and 7 years old were seen playing hide and seek along
the riverside, barefooted. Upon assessment, the 3 year old child weighs 9kg and appears pale; the children who are aged 5 and 7 years old are both
experiencing dry cough for 1 week without having any consultation with a physician.

The family lives in a 12 square meter by 20 square meter house of which the entire space accommodates the households’ kitchen, dining area, and
bedroom. In addition, the garbage is observed placed at the side of the house near the river bank with flies flying around while street dogs and cats
are also seen roaming around the neighborhood. The surrounding is unclean and the household area is apparently not suitable for living. One week
after the home visit, Nurse Mara makes a follow up visit to the family and interviewed the mother for the children’s immunization record. She found
out that 18month old baby XYZ has not received the first dose of MMR vaccine on her 9 th month. Likewise, mother XYZ has not received Tetanus
Toxoid shots.
