HOSPITALS TO THE EPIDETIOLOGY AND SURVEILLANCE UNITS TO AID I]II

All licensed government and non-govemment hospitals and health facilities are hereby Gminded
of their role in the reporting of notifiable diseases or health concem, as per Republic Aci No.
1 '1332, or the "Mandatory Reporting of Notifiable Diseases and Public Health Concern Acf .

To expedite information gathering for conducl of disease surveillance and epidemiologic investigation, all
health providers and heallh facilities are reminded to strictly and immediat€ly comply with the submission
and/or provide an easy a@ess of medical records and other pertinent records requested by the Regional
and Local Epidemiology and Surveillance Units-

Reporting of information to public health authorities by persons or entities mandated to report notifiable
diseases and heatth events of publac health concem, is rpt a violation of the Data Privacy Acl of 2012.

Further, all are enjoined to provide assistance and cooperate fully during tie epidemiologic investigation of
the Regional and Local Epidemiology and Surveillanco Units.

Failure of implementing units to comply with the reporting policies which include timely access to
personal information of individuals reported under the list Notifiable Diseases is PROHIBITED
under the Republic Act '11332 and its lmplementing Rules and Regulations.

This nemorandum shall serye as a document to access medical records and other pertinent
records.
