Monitoring and Evaluation Tool for the Months of January, February and March 2022

Directions: The results of this monitoring tool shall be used for the conduct of the proposed Training on
Technical Writing/Communication Skills and Personality Development for CID Personnel/School Heads.
This will aid the management to appropriately choose topics which will best help improve their
performances of the School Heads in the field. Kindly conduct a thorough assessment on the indicators and
answer them by ticking a column using following the scale from 1 to 5 where 1 is the lowest and 5 as the
highest. Your answers will be treated with confidentiality.

Indications

3. Are the bulletin boards, statistics and others posted
data updated and relevant?
4. Does the School/District Head provide some sections,
properly labeled as part of the office where important

the door with a Locator Chart and display his/her
monthly Reports? (Please check the monthly reports of
the School and District Heads and affix your signature

II. On Record and Report Management

3. Are the reports correct in terms of grammar, content,
style and form?
4. Does the School/District Head maintain of Logbook of
Incoming and Outgoing Communications?

III. On Communication Skills
1. Does the School/District Heads respond according to
Bilingual Education Policy (Question English,
Response English: Question is in Filipino 1 Answer in
Filipino) when giving instruction and casual
conversational activities?

2. Does the School/District Head speak according to


standards of good communication process?

3. Does the School/District Head speak according to


correct grammar and appropriate choice of words and
language functions?

4. Does the School/District Head convey/express the


message he/she wanted to put across?

5. Does the School/District Head cite examples which are


relevant to the topics for discussion?

IV. On Personality
1. Does the School/District Head follow Dress Code
Policy/In proper uniform?
2. Does he/she exhibit poise and good posture?

3. Does he/she exhibit good grooming and proper


decorum?
4. Does he/she exhibit appropriate entertainment of
standards for visitors?
5. Does he/she possess immediate and wise/sound
decision-making?
V. On School Management
1. Is the School clean, with ornamental plants and
signages, in and out of the rooms?

2. Does he/she respond to the health protocols as


evidenced by the facilities of the school, such as hand
washing, clean toilet, etc.?

3. Does the schools maintain a biometrics and a logbook


for School Heads and teachers and non-teaching
personnel?
4. Does the school implement PAPs of the
Division/Region and even Central Offices?

5. Are the rooms/clean/structured/painted/ and teachers


receive supplies/ Are the stock room empty, with
proofs of Books distribution?
