1.1 Admission Procedures and Requirements for incoming Freshmen, Transferees and Returnees 1.1.

1 Proceed to the Admissions Office for details as to choice of college program; (Please bring 1x1 picture); 1.1.2 Proceed to the Cashiers Office and pay the Admission Fee of three hundred pesos (Php 300.00); 1.1.3 Return to the Admission Office and present the Official Receipt and fill-up legibly and completely the Application Form; 1.1.4 1.1.5 Proceed to the Testing Room for the Qualifying-College Admission Test (Q-CAT); and Return to the Admission Office for the result of the examination.

If the applicant obtains the qualifying score for admission, he/she shall be advised to enroll by the Admission office. 1.2 The following are the Requirements for Enrollment:

1.2.1 For Incoming Freshmen a. Two 1x1 picture b. High School Report Card (original and photocopy) c. Certification of Good moral Character (original and photocopy) d. Birth Certificate (original and photocopy) 1.2.2 For Transferees and Returnees: a. Two 1x1 picture b. Honorable Dismissal/Transfer of Credentials (original and photocopy) c. Summary of Grades (original and photocopy) d. Certification of Good Moral Character (original and photocopy) 1.2.3 For Second Degree: a. Original and photocopy of Transcript of Record (TOR) b. Photocopy of Diploma c. Two 1x1 picture 1.2.4 Enrollment Procedure for Incoming Freshmen a. Proceed to the Admissions Office to submit the requirements for enrollment and for the schedule of classes b. Proceed to the Registrars Office for the encoding of subjects c. Proceed tot the Cashiers Office for the payment of fees d. Proceed to the ITC for the Identification Card

e. Proceed to the University Clinic for Medical and Dental check-up 1.2.5 Enrollment Procedure for Transferees and Returnees a. Proceed to the Admissions Office to submit the requirements for enrollment b. Proceed to your College interview and for the schedule of classes and pre-enrollment form c. Submit to the Admissions Office the pre-enrollment form duly signed by the Assistant Dean or his/her representative d. Proceed to the Registrars Office for encoding of subjects e. Proceed to the Cashiers Office for the payment of fees f. Proceed to the ITC for the Identification Card g. Proceed to the University Clinic for Medical and Dental check-up
