CONTRACT OF AGREEMENT
(Pakyaw for the Delivery of SBFP Commodities)

KNOW ALL MEN BY THESE PRESENTS:

This AGREEMENT, made and entered by this _____ day of _____________ 20____ at Isulan,
Sultan Kudarat by and between the School Health Section, DepED, Division Office, Brgy.
Kenram, Isulan, Sultan Kudarat represented by DR. HANNAH M. YTAC, RN and the Pakyaw
Labor Representative Mr. _____________________________________ of Brgy. ________________,
_____________________, Sultan Kudarat.

-WITNESSETH-
That for and in consideration of the payment for this Pakyaw Agreement, both
above parties hereby commit and agree as follows:

1. That Mr. _________________ will perform the following jobs:

1.1. Accept delivered commodities from School/District Inspection Team at designated

School Drop-Off point as scheduled
1.2. Segregate and deliver commodities as scheduled from School Drop-Off point to the
following assigned clustered sitio/purok:

1.3. Strictly observe proper hand washing, proper wearing of face masks and face shield
while rendering services of this contract.

2. That the DepED, Division Office will pay the total amount of
___________________________________________________ (P___________) as payment for the
labor.

3. Payment of labor shall be made after the inspection and acceptance of the work.
