provides women, men, and adolescents with a range of quality health services—from contraception and maternal
care to HIV prevention as well as AIDS care and treatment.
Global Fund project will support comprehensive HIV services in areas, namely Anti-Retroviral Treatment (ART),
Prevention of Mother-to-Child Transmission (PMTCT) programming, HIV Counseling and Testing (HCT), and
Integration of HIV and TB with Sexual and Reproductive Health (SRH) services.
Program Overview
Pathfinder International, as Sub-Recipient (SR) has signed a grant for the period ending December 2020 with the
Principal Recipient (PR), Family Health International (FHI360) funded by the Global Fund. Pathfinder in
collaboration with GoN will support comprehensive HIV services in areas, namely Anti-Retroviral Treatment (ART),
Prevention of Mother-to-Child Transmission (PMTCT) programming, HIV Counseling and Testing (HCT), and
Integration of HIV and TB with Sexual and Reproductive Health (SRH) services in North West Nigeria
Position Purpose
The case manager will support clients' care by assessing treatment needs; support the clients to develop, monitor,
and evaluate their own treatment progress and facilitate interdisciplinary approaches including but not limited to
Index Case testing, improving referrals and linkages to much needed services within the program.
Basic Requirements
First degree or equivalent in the sciences, OR Minimum of S.S.C.E/NECO who have previously held an
equivalent position within the hospital setting.
Case managers should be literate in both English language and the local dialect.
They should preferably be expert clients and/or social workers, retired health workers who have relevant
knowledge and skills: Documentation Skills, Decision Making, Verbal Communication, Written
Communication, People Skills, Resolving Conflict, People Management, Integrity, Connecting with clients
with chronic diseases.
Residence in the communities/LGA where the health facility is domiciled and be familiar with the
environment is an added advantage
Mature, responsible and able to meet deadlines under pressure and work with minimal supervision
Ability to deliver within tight deadlines and prioritize workload.
