SUBJECT : SUBMISSION OF REPORTS ON INCIDENTS OF CHILD ABUSE, BULLYING;

CASES INVOLVING CHILDREN-AT-RISK AND CHILDREN-IN-CONFLICT-
WITH-THE-LAW; AND CHILD PROTECTION AND ANTI-BULLYING
POLICIES PURSUANT TO DEPED MEMORANDUM NO. 114, S. 2017

Pursuant to DepEd Memorandum No. 114, s. 2017, Public and Private schools, both
elementary and secondary schools, shall submit the following to their respective SDOs the following:

a. Accomplished Report on Incidents of Bullying

(Enclosure No. 1 of DM 59, s. 2015);
b. Accomplished Report on Incidents of Child Abuse
(Enclosure No. 1B of DM 59, s. 2015);
c. Accomplished Report on Cases Involving CAR and CICL in Schools (Appendices
D and F of DO 18, s. 2015);
d. An updated copy of the School-Based Child Protection Policy and Anti-Bullying
Policy certifying their adoption and localization of DepEd No. 40, s. 2012;
e. Report on the Status of their Established Child Protection Committee.

Please see attached forms.

In line with this, all concerned are expected to submit the following reports for S.Y. 2019-2020
on or before 10 JULY 2020 at 5:00 in the afternoon in PDF format to be sent through electronic mail
at rowell.malabag@deped.gov.ph. Original copies of such reports shall remain in custody of the
School Heads/CPC Coordinators.

PSDSs shall ensure that the schools under their respective supervision comply with the
abovementioned Memorandum.

Failure to submit the aforementioned reports and policies within the period provided shall also
require submission of written explanation.
