AFFIDAVIT to SUPPLY FIRST NAME

I, ARLENE MAGLENTE SOLIDOR, Filipino, of legal age, married and a

resident of Zone 2, Pago, Tanauan, Leyte under oath declare:

1. That I am the mother of RUSTICO, JR. MAGLENTE SOLIDOR who was

born on 23 December 2001 at Tacloban City;

2. That my son’s birth was registered with the Local Civil Registry, Tacloban
City, but no entry was made on the first name of the child in his Certificate
of Live Birth;

3. That in his other pertinent documents and records, his complete name
appears therein as RUSTICO, JR. MAGLENTE SOLIDOR;

4. That I am executing this affidavit to attest the truth of the foregoing facts
and for purposes of supplying my son’s first name as “RUSTICO, JR.” in
his Certificate of Live Birth at the Civil Registrar, Tacloban City.
