Prestart-up
A PHA should be done before the start-up of the process. It is appropriate at
this stage to go over the existing PHAs that have been conducted and to
update them. Consideration should also be given to changes in the design
that have been made since the design review and changes that have been
made to the operating concepts now that procedures are firm. The prestart-up
