RESERVATION OF MEDICAL RIGHTS AND REQUIREMENTS

Due to past gross violations of tyranny, abuse and violations by the medical and pharmaceutical
industries and government agencies and officials, we must now, and hereby do require certain
compliance and performance by any and all medical care providers with regard to the identified
patient/individual below.

1. Individual rights and demands are the supreme and primary policy control for the
administration of medical services.

2. Medical treatment is always voluntary for the patient and seeker of medical care and/or
information.

3. Medical treatment is always private. No databases or records are shared without disclosure to
and consent of the patient.

4. Medical care providers who directly examine and consult with the patient are the sole and final
points of recommendation for treatment for their patients, and patients are the sole and final
authorities over what treatments and programs they will receive.

5. Payment will be direct from the patient to the medical provider and no third party will control,
administer or be informed of any such payment. If payment is arranged by any third party, that
third party shall have no authority over medical decisions of the patient or the health care
provider.

6. No medical treatment will ever be "mandatory" or "required" at any time for any reason.

7. No treatment or medical record will ever be required to be reported to any government,

corporation, person or entity not expressly approved by the patient.

8. No medical provider shall be held to perform to any pre-determined or coerced "protocol".

9. Patients and individuals who stipulate this independent system of medical care are declaring, by
their use of it, that other forms of medical care are or may be unlawful, unwanted, unsafe,
unreliable and not regarded as legitimate and may not, under any circumstances, be imposed on
anyone by any method.

10. All Pandemics and epidemics will be resolved by informing the public of any perceived risks
and events and then letting individuals and medical care providers develop their own responses.
Central authority shall never be applied to medical care in the form of any force, coercion,
requirement or mandate, and if such attempt shall be made, that attempt shall be disregarded by
the patient and the health care provider.

11. The political, financial and ideological corruption, prejudices and interests of central authority
shall never be allowed to alter or influence the medical decisions of any individual or medical
service provider.
 12. The medical care provider who is directly examining and consulting the patient shall be the
only entity allowed to recommend treatment. That medical care provider shall never insist on,
"require" or coerce any treatment upon a patient.

13. The patient has absolute right to know the full and true ingredients and processes of any
treatment, the efficacy of that treatment, the potential side effects and harm that may be caused
by that treatment, and the record of injury and illness caused by that treatment. On that
information the patient shall have absolute right of refusal of any treatment based entirely on
the patient's wishes, expressed or implied.

14. No patient or individual may be required or coerced to receive a treatment that has been
"proven safe" or "necessary for the public good". The patient always has right to refuse any
treatment regardless of safety or public interest. Public interest, if to be protected, must be
protected by methods that do not violate individual right of absolute authority over medical care
which he or she receives.

15. Experimental or unproven treatments shall be disclosed as such and affirmatively represented as
hazardous and voluntary.

16. Included in the requirement of voluntary participation are garments such as any masks and
protective equipment that have not been shown to be perfectly effective and without risk or
harm to the person using such equipment.

17. Under no circumstances will I or any of my family members be refused access to each other. No
medical service provider or institution may isolate or quarantine

18. No medical services or products will be paid for which are not disclosed in advance and for
which affirmative approval has not been given to the medical service provider by the payer
prior to commitment.

By accepting the individual below as a patient, all medical service providers agree to the above terms,
positions and declarations. This notice, upon presentation, serves as that agreement by clear expression
of requirements and is forceful upon presentation and/or delivery. Ratification by the receiver is not
necessary.

The person or patient who requires and demands that ALL principles above are fully recognized and
