GROCERY LOAN AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Agreement is entered by and between:

_______________, of legal age, Filipino, single and a resident of Block 22, Lot
17 Chrysanthemum St. Pinewood Subdivision, Barangay San Jose, Malaybalay
City, Bukidnon 8700 (hereinafter referred to as “DEBTOR”);

-and-

________________, of legal age, Filipino, single and a resident of Block 7, Lot

12 Greentown Villas 1 Ext. Annex Barangay Mambog III, Bacoor City, Cavite
4102(hereinafter referred to as “CREDITOR”);

WITNESSETH: That-
WHEREAS, the DEBTOR has an existing obligation to the CREDITOR for an amount of P_____ with an
interest of __________, with the total amount due of ________as a payment to the GROCERY loan
____________________;
WHEREAS, for and in consideration of the foregoing premises, the parties hereto agree as follows:
1. The PARTIES hereby agree to the scheduled PAYMENT PLAN.

2. The DEBTOR shall conform to the schedule set and shall pay to the CREDITOR before or upon due
the amount as indicated in the PAYMENT PLAN.

3. Payment shall be made to the CREDITOR thru ___________________.

4. If for any reason the DEBTOR should not obligate to pay the total amount or delay of his/her payment
on the due date of this Agreement, the DEBTOR shall be considered in default. Under such an event,
the remaining balance of the Amount Owed shall be due and demandable with an interest of ______
within up to 10 business days only.

5. If for any reason the DEBTOR should not obligate to pay the total amount with the default interest
within 10 business days upon default. The CREDITOR with due notice to the DEBTOR, shall file legal
action.

IN WITNESS WHEREOF, the Parties have executed this Agreement, on this ____ of _________ at
Malaybalay City, Bukidnon.
