AFIDAVIT OF DISCREPANCY
I, A, of legal age, Filipino, married and with residence and postal address at
Sitio Calawang, Calapan City, after having been sworn to in accordance with law,
hereby depose and state:

1. That, nine years ago, I have registered an account under my name with
the Philippine Overseas Employment Administration(POEA);

2. However, due to some system changes of the POEA, I was compelled to

create a new account which resulted to my new account bearing an
Account Number xxxxx;

3. That, during the transfer of my information from old account to new

account, I discovered that my birthday was erroneously indicated to be
“28 September 1983” instead of “28 August 1983” in my old account;

4. That, in order to correct the same, I do hereby attest that my correct and
true birthdate is “28 August 1983” as evidenced by my Philippine
Passport and my LTO Driver’s License. Copies which are likewise
attached herewith for reference as ANNEX ”A”.

5. That this affidavit is being executed to attest to the truth of the foregoing
statements and for whatever legal purpose this may serve.
