(SGOD) issues this addendum to Unnumbered Division Memorandum
dated June 16, 2022 relative to the inclusion of the following:

A Division Monitoring Team
B School Monitoring Form
C District Consolidated Monitoring Form

2. All other provisions of Unnumbered Division Memorandum dated June

16, 2022 shall remain.
