ORDER

A verified petition has been filed before this court by the petitioner thru counsel praying
that after due notice, publication and hearing, a decision be rendered directing the Local Civil
Registrar of San Fernando City, La Union to change the entry in item 1 referring to the first
name of the petitioner from Coco to Caca and item 2 referring to the sex of the Petitioner from
Male to Female.

Petitioner prays for such reliefs as are just and equitable under the premises.

Finding the petition to be sufficient in form and in substance, the court hereby sets the
initial hearing on ___________________________ at which time, date and place, all persons
interested in this Petition shall appear before the REGIONAL TRIAL COURT BRANCH ___,
San Fernando, La Union and show cause why said Petition should not be granted.

Let copies hereof and of the Petition be furnished the Office of the Solicitor General,
Makati City, the Public Prosecutor San Fernando City, La Union, the Petitioner and the counsel.

Further, let this be order be published at the expense of the Petitioner once a week for
three (3) consecutive weeks in ______________________ (as raffled) edited and published in
the Province of La Union.

the Public Prosecutor.
