In order to complete your enrollment in the Assurance Wireless Unlimited Plan, you must review
and accept the Assurance Wireless E-Signature Consent terms below.
Consent to Use Electronic Records and Signatures. You agree to accept all communications
electronically and use electronic signatures ("Consent") with Assurance Wireless USA, LP. and our
controlled subsidiaries, assignees, agents and participating financial institutions and any entity assigning
an Equipment Installment contract to us. Unless you agree, we are required to provide some
Communications to you in writing.
Your Consent applies to your entire relationship with us, including this transaction. Unless you
withdraw your consent, your agreements with us, equipment installment plan contracts, lease
agreements, monthly statements and any disclosures, notices or other information about your
relationship with us ("Communications") will be delivered electronically. We may, if we choose,
require Communications to be in writing.

your email and contact preferences whenever you want by visiting your account at
www.assurancewireless.com or calling us at (888) 321- 5880. All Communications provided prior
to your withdrawal of Consent are still valid. After you withdraw Consent, your future
Communications with us will be in writing whenever required by law.
You can get a paper copy of any electronic Communications at no charge. Identify the document

(including PDF reader), email and SMS access to receive Communications electronically. You represent

T-Mobile Essentials, Metro by T-Mobile) may notice lower speeds than other customers due to data prioritization.

If you change your mind after you are approved for Assurance Wireless Lifeline service, you can contact
Customer Care at 1-888-321-5880 to enroll in Assurance Wireless Unlimited Plan.
• I acknowledge that I may obtain broadband service supported by the Affordable Connectivity
Program from any participating provider, and I may transfer my ACP benefit to another qualifying
ACP provider only once per service month unless an exception applies.
• I acknowledge that I will be subject to the Assurance Wireless general terms and conditions and
moved to a Lifeline-only service if eligible, or otherwise to a Pay As You Go plan if ACP ends, if I
transfer my benefit to another provider but continue to receive service from Assurance Wireless, or
upon de-enrollment from ACP.
• I acknowledge that I may file ACP-related complaints against Assurance Wireless through the
Federal Communication Commission’s Consumer Complaint Center.
• I acknowledge that I have been informed about Assurance Wireless’ ACP-supported service plans
that are fully covered by the applicable affordable connectivity benefit amount.
• If I currently receive an ACP benefit from another carrier, I request that Assurance Wireless submit
a Benefit Transfer on my behalf. I understand that I will only receive the ACP benefit from
Assurance Wireless and will lose any benefit from my current service provider. I understand that I
may be subject to my previous provider’s undiscounted rates as a result of the transfer if I keep
service from my previous provider. I also understand that I am limited to one ACP-transfer
transaction per service month unless an exception applies. I acknowledge that I was provided the
foregoing information about the transfer of my ACP benefit, and I consent to transfer my ACP
benefit to Assurance Wireless.
• I authorize government agencies and their authorized representatives to discuss with, receive from,
and provide information to Assurance Wireless that is relevant to my eligibility to receive ACP from
Assurance Wireless.
• I give my consent to Assurance Wireless to use and transmit my personal information, including my
name, address, date of birth, last four digits of my SSN or my Tribal ID number, my telephone
number, and other personal information, as well as the amount of the discount I receive, the date I
was enrolled in and the date I was de-enrolled from the Affordable Connectivity Program with
Assurance Wireless, as necessary, to verify my eligibility to receive the Affordable Connectivity
Program benefit with the Universal Service Administrative Company.
The Affordable Connectivity Program (ACP) is a government program that reduces the customer’s
broadband internet access service bill. One discount per eligible household and is non-transferable across
households. An eligible household may obtain ACP-supported broadband service from any participating
provider of its choosing and may apply ACP benefit to any broadband service offering of the participating
provider at the same terms available to households that are not eligible for ACP-supported service. ACP
service provider may disconnect the household’s ACP-supported service after 90 consecutive days of non-
payment; the household will be subject to the provider’s undiscounted rates and general terms and
conditions if the program ends, if the consumer transfers their benefit to another provider but continues to
receive service from the current provider, or upon de-enrollment from ACP. A household may file a
complaint against its provider via the FCC’s Consumer Complaint Center,
https://consumercomplaints.fcc.gov; visit https://acpbenefit.org for more information on ACP. Unlimited
