Official Konsulta Tarpaulin Banner Designs

for Accredited Konsulta Providers
In line with the implementation of the Konsultasyong Sulit at Tama (Konsulta)
Package, please be advised that all accredited Konsulta providers are required to
display a signage indicating that it is an accredited Phil Health Konsulta provider
(Annex A). This is in addition to the "New PhiiHealth Signage" as provided in
Phil Health Circular No. 0013 s. 2009.

Per Annex B (Self-Assessment/ Accreditation Survey Tool) of Phil Health Circular

No. 2020-0021 titled "Accreditation of HCPS for Konsulta Package;' the signage
indicating that the facility is a PhiiHealth Konsulta provider is among the
minimum requirements for an accredited Konsulta provider. The signage shall
be available within three (3) months from approval of initial accreditation.

The following are the technical specifications and the links for the tarpaulin
designs:

Frame Material: Aluminum Size of tarpaulin: 4ft. x 2.5 ft.
Type: Roll-up Standee Thickness: 13 oz.
Size of tarpaulin: 2.75 ft. x 6.5 ft. Others: with seaming and four (4)
Tarpaulin thickness: 10 oz. corner eyelets
