ITER_D_2F6B9M v2.2 ITER_D_2F6B9M v2.0 When performing a HAZOP it is important to define the consequences of interest which are the level or degree of harm that is unacceptable to personnel, the public, the environment, the property, operations, or corporate image. Conditions resulting in consequences of interest must be controlled further to reduce the risk. Before beginning your HAZOP study, your team should decide appropriate consequences of interest. Examples are: compromise confinement to environment or workers (second and first confinement), fire, explosion, no path forward waste and loss of system availability. Certain specific consequences of interest have been identified by ITER which require specific documentation. At a minimum the following consequences of interest will be used. Consequence of Interest Hazard ~ Compromised confinement to Environmental Release Radiological environment (second) + T; present in system where not planned + Compromise confinement to workers Worker Exposure Radiological (first) + Radiation impact on worker + Ingress/Egress issue that will affect worker + Breach of chemical confinement to Environmental Release Chemical environment + Breach of chemical confinement to Worker Exposure Chemical worker + Possible combustible mixture Fire + Exceed combustible limits + Oxygen leak into system, possible combustible mixture + Smoke to adjacent location + Hy above Lower Explosive Limit, 4% Explosion (LEL) + Flammable Gas above LEL + Oxygen leak into system LEI H, above: + Formation of H;0() that will interact Corrosion’ with metal in system + Introduction of dust into system that will result reaction with metal confinement + Introduction of other corrosives + Mechanical event could cause harm to | Worker Injury (Mechanical/Physical) worker © Projectile o Thermal + Production of no path-forward waste Increased Waste Production + _Inereased production of effluents waste + System Unavailable RAMI + Maintenance
